A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Alcohol test strips are expired. Procure new alcohol test strips and provide to attending marine inspector.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Two life jackets stored at the operating station stowage was not marked. Mark stowage in 2 inches lettering with Adult life jacket and quantity to the satisfaction of the attending marine inspector.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel missing abandon ship drills logs and descriptions and scenarios for both drills. Provide drill log for abandon ship and MOB drills with descriptions to the satisfaction of the attending marine inspector.

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Fire drill logs were missing. Provide logs and required drill descriptions to the satisfaction of the attending marine inspector.

ach lifejacket must be approved under approval series in subparts 160.002, 160.005, 160.055, 160.115, 160.176, or 160.255 in part 160 of this chapter, or other standard specified by the Commandant. One of the required 32 life jackets was missing. Provide one new CG approved life jacket. Vessel passenger count reduced to 28 passengers until life jacket is provided to meet the COI required 32.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol test strips provided on board the vessel were expired. Provide current alcohol testing devices.

Oceans, coastwise, limited coastwise, and Great Lakes routes. A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry—(1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and(2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel only had (3) valid distress signals on board the vessel. Vessel provided required (12) distress signals to the attending marine inspector. Deficiency Rectified.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The hydrostatic release unit was found expired in 2022. Provide current hydrostatic release unit.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit contained expired analgesic medication and antiseptics. Provide current contents.

A hydrostatic release unit when used in a float-free arrangement must be approved under approval series 160.062 or 160.162 or other standard specified by the Commandant. Manufacturer requires replacement 2 years after installation. Hydrostatic release was marked as expiring in 2021. Replace hydrostatic release to facilitate float free operation.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Aspirin was expired. Provide complete and current kit.

The Rules concerning shapes shall be complied with by day. The lights and shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84). Vessel was not carrying day shapes.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. FCC Safety Radiotelephony Certificate was found expired. Provide valid FCC Safety Radiotelephony Certificate.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Current publications were not available on board. Provide current Coast Pilot [name withheld], Tide and Currents.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in §185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions were not found on board. Provide and post emergergency instructions.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child sized life jackets were found stowed with and intermixed with adult life jackets. Vessel provided a bag to separate life jackets.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Abandon Ship and Fire logs were not available during inspection. Drills conducted during inspection were logged by master.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Logs showing monthly test were not available. Master logged test conducted during inspection.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see §175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Records of monthly checks of portable fire extinguishers were not available. Master logged visual inspections completed during annual inspection.

Vessel suffered a leak in the port engine transmission oil cooler resulting in a discharge of oil and a reduction in propulsion. Repair and demonstrate proper operation to attending USCG Marine INspector.

Prior to operating beyond 3 NM, replace expired EPIRB hydro release.

Prove proper operation of SeaFire engine shutdown override.
