The Master, owner, operator, agent or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following methods: Install and operate a ballast water management system that that has been approved by the Coast Guard. Following installation, the Master, owner, operator, agent or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. The Coast Guard approved BWMS must meet the applicable ballast water discharge standard (BWDS)found in 151.2030 of this subpart at all times of discharge into the waters of the United States. The vessel's BWMS was reported inoperable. Vessel has parts orderd for the Port of New York.

The safety of passengers, crew, and ship from electrical hazards will be ensured. PSCO observed missing panel with exposed wiring on No 2 HFO Purifier Control Panel. SOLAS(04) II-1/40.1.3

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed spraying misting water from steam pipe. SOLAS II-1/33.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed 11 missing pins on CO2 bottles.

Fire-fighting systems and appliances shall be kept in good working order and readily available or immediate use. PSCO observed 06 missing pins and 01 improperly placed pin on Co2 bottles compromising operational readiness of the Co2 fixed fire fighting system.

Hydraulic power-operated steering gear shall be provided with a fixed storage tank having sufficient capacity to recharge at least one power activating system including the resevoir. Reserve hydraulic oil storage tank was 50% full, thus, not meeting the requirement prescribed by administration.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, ignition sources shall be restricted. Accumulated oil and oily wrags were found under 04 generators.

A lifejacket complying with the requirements of paragraph 2.2.1 or 2.2.2 of the code shall be provided for every person on board the ship. Lifejackets on board found not to be in compliance with listed regulation.

0715-Fire Detection- Any required fire detection and fire alarm system shall be capable of immediate operation at all times and no action by the crew shall be necessary to set it in operation. Fire detection in lower deck port side engine room did not immediately set off fire alarm system upon testing in presence of PSCO. 74SOLAS(01)II-2/12.1.1 17ac

0820-Protection machines/parts-All surfaces with temperatures above 220degC which may be impinged as a result of a fuel system failure shall be properly insulated. #1 turbo exhaust manifold on main engine was missing sections of protective lagging in two locations on turbo; 2'x4" section and 1'x4" section. 74SOLAS(01)II-2/15.2.10 17ac

0830-pipes/wires(accident prevention) All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed exposed low voltage wires by way of port forward jacket water preheater unit on the third engine room deck. 74SOLAS(04)II-1/45.1.3 17ac

0750 - Doors require to be self-closing shall not be fitted with hold-back hooks. Galley doors are all fitted hold-back hooks. 74 SOLAS 2001 CONS. II-2/47.2

VSL failed to fully implement the SMS. VSLS SMS 12.4.1.6 all containers should be well secured before the vsl leaves port. Vsl failed to properly secure twist locks on containers. Vsls SMS 12.1.7 states when heavey sea is encountered addl securing means are to be provided. While encountering swells vsl lost 3 containers over the side. SMS External audit rec 74SOLAS(09) IX/3.1

Efficient guard rails shall be fitted to all exposed parts of the freeboard and super-structure decks. STBD side guard rail near bay 42 sustained significant damage cause by falling container. ICLL(66) Reg 25.2
