Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Insulation on MDE exhaust needs replacement.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Vessel must carry U.S. Coast Pilot [name withheld] List on board.

A COI for a towing vessel is valid for 5 years from the date of issue. Vessel's COI is expired and restricted from operations until a COI inspection is completed.

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Air receiver in engine room missing indication of MAWP.

Relief valves must be tested in accordance with § 143.245. No pressure relief valves were installed on pressure vessel.

Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container that meets the requirements of § 142.225 of this subchapter. Flammable and combustible products were found stored in engine room.

Each towing vessel must be fitted with a general alarm that: Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”. Sign missing in engine room.

The following systems must be equipped with gauges at the machinery location:(1) Main engine lubricating oil pressure and main engine RPM;(2) Main engine cooling water temperature. Required gauges at machinery location missing.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided:(1) Main engine low lubricating oil pressure;(2) Main engine high cooling water temperature. No alarms for engine oil pressure or water temperature installed.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections were not documented.

A navigation assessment entry must be recorded in the TVR, official log, or in accordance with the TSMS applicable to the vessel. The entry must include the date and time of the assessment, the name of the individual making the assessment, and the starting and ending points of the voyage or trip that the assessment covers. No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. Each lifejacket must be marked: With Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16) (incorporated by reference, see § 136.112 of this subchapter). Retroreflective material on life jackets found waisted.

All lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, and identity, of lifejackets stowed inside the container. Life jacket container in pilot house missing label.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). Vessel equipped with VHF-DSC radio but MMSI number is not programmed into radios.

A towing vessel whose construction was contracted for prior to January 18, 2000, may use an existing engine room monitoring system (with fire-detection capability) instead of a fire-detection system, if the monitoring system is operable and complies with paragraphs (a)(2) through (7) of this section, and uses detectors listed by an NRTL. Engine room monitoring system fails to meet the following requirements: (4) The control panel includes:(v) A circuit-fault detector test-switch, or internal supervision of circuit integrity (5) The system draws power from two sources. Switchover from the primary source to the secondary source may be either manual or automatic. Once corrections are made, have engine room monitoring system inspected and certified by a registered professional engineer with experience in fire-detection system design, or a technician with qualifications as a as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society.

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by § 136.202. 46 CFR 136.200(a) COI is invalid. Vessel missed annual inspection. Must complete prior to operating.
