Vessel was unable to demonstrate operation of fuel transfer automatic shutoff valves due to valves being damaged. Vessel is required to demonstrate automatic and manual operation of all fuel shutoff valves to the satisfaction of USCG Marine Inspector.

Emergency lighting must be arranged so that enough lights throughout the machinery space to allow essential operations and observations under emergency conditions to allow restoration of service. Lighting for escape trunks in machinery spaces to allow crew to find their way to open decks and survival craft. MI's observed insufficient lighting in the engine room. Vessel is to install/ensure lighting is sufficient for crew to egress from machinery space. Install an additional light at the emergency escape trunk and identify a clear path to the escape trunk from all areas of engine room. Lighting in way of engine room door and switchboard was insufficient, install or ensure that lighting is sufficient in these areas. Installation is to be to the satisfaction of USCG marine inspector.

The Class I quick acting watertight door located forward of the main engine room machinery space requires excessive force to secure properly and must be adjusted to allow an appropriate amount to force for each crewmember to safely and adequately secure it. Adjust watertight door to allow for all crew members to close the door.
