Resolution: The Company has obtained technical advice from the maker's rep who informed the C/E the procedure is to set the appropriate set point of the M/E governor. The adjustments were made by the C/E and were confirmed by the attending class surveyor.

Resolution: Interviews from the surveyor revieled that an order was given to switch air bottles as the air pressure became low. The crew member opened the #1 bottle discharge valve but did not close the #2 bottle 1st resulting in loss of air pressure. See narrative.

Doors required to be self closing shall not be fitted with hold back hooks. The door into the ladder well on the upper deck is self closing but is fitted with a hold back hook. An additional hold back was discovered on the emergency escape ladderwell coming from the engine room. 74 SOLAS(04) II-2/9.4.2.2

These measures should include the regular testing of stand by arrangements and equipment. Engineering crew was unable to properly test smoke detector in engine room upon request.
