PSCO observed jacket cooling water leaking from M/E cylinder #7 head gasket 74SOLAS (04) II-1/26.1

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed excessive leakage from emergency fire pump packing gland and smoke after crew made adjustments 74SOLAS(24) II-2/14.2.1.2

Guard rails fitted on superstructure and freeboard decks shall have at least three courses. The opening below the lowest course of the guard rail shall not exceed 230mm. The other courses shall not be more than 380mm apart. PSCO observed broken sections of guard railing on deck near cargo hold 2 and 4 portside and near cargo hold 4 on starboard side ICLL (88 prof) Reg.25(3)

Every ship of 100 gt and above... shall carry a garbage management plan which the crew shall follow. PSCO observed crew storing garbage in upper deck escape route with no intention to remove garbage at the berth/ terminal. MARPOL V/10.2

Escape routes shall be maintained in a safe condition, clear of obstacles. PSCO observed escape route on upper deck obstructed by excessive garbage. 74SOLAS(09) II-2/13.1.2

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. PSCO observed 02 inoperable emergency lights around superstructure 74SOLA(04) II-1/ 40.1.2

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met: Fire protection systems and firefighting systems and appliances shall be maintained and ready for use. PSCO observed the quick closing valve on the # 2 generator is missing a nut and the plunger was had excessive vibration. When addressing the Chief Engineer about testing the #2 quick closing valve, the Chief Engineer disassembled the air supply line to the #2 quick closing valve in order to test. No maintenance procedures or records could be provided to any of the generators quick closing valves. 74 Solas (2020) II-2/14.1.1 60ac

The arrangements for the storage distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure that safety of the ship and persons onboard. PSCO observed the engineer cleaning the deck below the pneumatic directional control valve for the main engine where it was found leaking. 74 SOLAS (2020) II-2/4.2.3.1 60ac

The conditions of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. PSCO observed approximately two feet of water in the bilge pockets. It was found that multiple packing glands on pumps and valves were leaking excessively. 74 SOLAS (2020) I/11(a) 60ac

PSCO observed the bosun hole had excessive gear adrift. 74 SOLAS (2020) I/11(a) 17ac

Every ship of 100 Gross tonnage and above and every hip which is certified to carry 15 or more persons and fixed and floating platforms shall carry a garbage management plan which the crew shall follow. The plan shall provide written procedures for minimizing, collecting, storing, processing, and disposing of garbage including the use of equipment on board. It shall also designate the person or persons in charge of carrying out the plan such a plan shall be based on the guidelines developed by the organization and written in the working language of the crew. PSCO observed bags of oily rags and paint on the outboard side of the cylinder oil settling tank and the lube oil settling tank. Additionally, there were buckets of oil in containment on the port side bunkerline containment. Garbage was notable all throughout the deck walk ranging from stern, cargo deck, and bosun hole. Doors required to be self-closing shall not be fitted with hold back hooks. However, hold back arrangements fitted with remote release devices of the fail safe type may be utilized. PSCO observed the two A-60 boundaries to the steering gear compartment lodged open with a pole and the other was tied open with a line. Additionally, on the third deck port side door was found with a line tied to it. 74 SOLAS (2020) II-2 9/4.2.2 10 c

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed inadequate pressure from forward fire hose as well as an excessive leak and smoke when testing the emergency fire pump. 72 SOLAS (cons) ||-2/14.2.1.2, 60ac

Every inflatable life raft...shall be serviced at intervals not exceeding 12 months, provided where is any case this is impracticable, the administration may extend this period to 10 months. PSCO observed that documentation could not be produced showing that an annual life raft inspection was completed as well as the chief officer stating they do not have the testing kit available on board. 72 SOLAS (cons) |||/20.8.1.1, 17ac

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed, and installed as to withstand the maximum working stresses. PSCO observed water/steam leaking from the water fill valve running down insulation and steaming off. 72 SOLAS (cons) ||-1/33.1, 50ac

The company should establish procedures to ensure the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of deficiency 01, 02, and 03, issued on 24 September 2025, provide objective evidence of a failure, or lack of effectiveness, of the implementation of the ISM case. The arrangements for the utilization of oil used in pressure lubrication systems shall be such as to ensure safety of the ship and persons on board. PSCO observed excessive oil leak from turbo oil discharge line on #2 generator. 74 SOLAS (04) II-2/4.2.3.1

The company and the ship shall comply with the requirements of the ISM Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The company should ensure: appropriate corrective action is taken. PSCO observed, during an expanded exam, the following: failure of the company and the ship to properly manage failing equipment, to include: INMARSAT-C, emergency fire pump, sanitary accommodations, and maintain proper security records. An external ISM audit is required prior to release from the detention. 74SOLAS(24) IX/3.1 ISM Code 10 30abc

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines. PSCO observed that the Master and [name withheld] (the designated Ship Security Officer) were not familiar with proper security exercise and drill record keeping. 74SOLAS(24) IX/3.1 ISM Code 6.4 17abc

Every inflatable life raft… shall be serviced: at intervals not exceeding 12 months… PSCO observed that for the port, starboard, and forward life rafts, which were of the extended servicing type, the ship did not have any record of shipboard servicing at the 12 month interval. 74SOLAS(24) III/20.8.1 17ac

The machinery… and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended… PSCO observed a significant leak from an expansion joint in the fire main in the BOSN store. 74SOLAS(04) II-1/26.1 17ac

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed that the emergency fire pump was mis-configured, resulting in damage to the packing gland which burned and caused the space to fill with smoke. Once the packing gland had been loosened, the emergency fire pump leaked excessively. 74SOLAS(09) II-2/14.2.1.2 17ac

PSCO observed that the control for the fixed-foam extinguishing system was set to manual operation, which was contrary to ship’s procedures, and was not available for immediate use. 74SOLAS(09) II-2/14.2.1.2 17ac

Sanitary accommodation shall comply with the following requirements: water closets provided with an ample flush of water, available at all times and independently controlled. PSCO observed that three toilets in the B Deck lavatory that did not flush. PWSA 46 U.S.C. 70002 ILO 147, pg. 37 / 21(f) 50ac

The machinery, boilers, and other pressure vessels and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed open tank top to cascade tank, exposing hot surfaces and steam, and presenting a danger to persons on board. 74SOLAS(04) II-1/26.1 50ac

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the pan's implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. The plan must include - actions for implementing the mandatory BWM requirements and practices. PSCO observed that procedures for using the installed BWM system were not implemented in the vessel's BWM plan. 33 C.F.R. 151.2050(g)(2) 40ac

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I, so that all entries in the book appropriate to that operation are completed. PSCO observed that the Chief Engineer had not completed an internal tank transfer entry in the Oil Record Book without delay, as required. Chief Engineer made corrections to Oil Record Book during exam. MARPOL I/17.4 10c

Sanitary accommodation shall comply with the following requirements: cold fresh water and hot fresh water available in all wash spaces. PSCO observed that no wash spaces onboard the vessel had hot fresh water available. Hot water was restored during exam. PWSA 46 U.S.C. 70002 ILO 147, pg. 37 / 21(c) 10c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed twisting movement of the steering ram while conducting steering test locally and control from the bridge. 74SOLAS(24)I/11(a) 60ac

Bilge wells in periodically unattended machinery spaces shall be located and monitored in such a way that the accumulation of liquids is detected at normal angles of trim and heel, and shall be large enough to accommodate easily the normal drainage during the unattended period. PSCO observed forward port bilge alarm in engine room inoperable.74SOLAS(04)II-1/ 48.1 17ac

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part 1, so that all entries in the book appropriate to that operation are completed. PSCO observed that OWS overboard discharge recordings that were completed between 15MAR25 and 29MAR25 in the ORB did not match specifications of the OWS maximum throughput in the IOPP Supplement, and several entries had incorrect usage of codes. The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish and maintain procedures for identifying any training which may be required in support of the safety management system and ensure that such training is provided for all personnel concerned. PSCO observed during an expanded exam the following; failure of the crew to properly operate the OWS within the required maximum throughput amount due to lack of familiarity with equipment on board, and lack of operational readiness following testing of the watermist system in the purifier space. An internal ISM audit is required within 3 months of the date of the exam. 74SOLAS(24)IX/3.1 ISM Code 6.5

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed plastic bags covering watermist nozzles in purifier space over fuel oil purifiers that were not removed after previous system tests. 74SOLAS(24)II-2/ 14.2.1.2 10c

Doors fitted in boundary bulkheads of machinery spaces of category "A" shall be reasonably gas tight and self-closing. PSCO observed that the fire doors in the boundary bulkhead and in the escape trunk in the engine room did not self-close and latch shut. 74SOLAS(09) II-2/9.4.2.1

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire stations enclosed by "A" class divisions, and sections enclosed by "B" class divisions. PSCO observed that doors to the fire locker and to the galley were not self-closing A-60 boundary doors as required by the fire control plan. 74SOLAS(09) II-2/15.2.4.1

Means for controlling flammable liquids in a space shall be provided. PSCO observed excessive pooling of flammable liquids at F.O. circulation pumps #1 and #2. 74SOLAS(09) II-2/5.1.2

The boilers, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are inteded and shall be so installed and protected as to reduce to a minimum the danger to personnel on board. PSCO observed steam leaks at two locations on the boiler. 74SOLAS(09) II-2/26.1

Surfaces with temperatures above 220 degrees C which may be impinged as a result of a fuel system failure shall be properly insulated. PSCO observed damaged lagging on the F.O. return lines from each cyliner of the Main Engine. 74SOLAS(09) II-2/4.2.2.6.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable harm to the marine environment. PSCOs noted that the vessel's incinerator is inoperable. MARPOL Annex I Reg. 6.4.1

The master, owner or operator is responsible for removing fouling organisms from the vessel's hull on a regular basis. PSCOs observed significant marine growth on hull near and below the waterline. 40c
