0635 - Launching arrangement for rescue boats - Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. During a lifting test of the ships rescue boat, PSCOs observed that the boat would not lift high enough above the cradle to launch safely.

2055 - Manuals, instructions, etc. - The training manual shall contain instructions on the life-saving applicances provided in the ship and on the best methods of survival and shall include donning of immersion suits. PSCOs observed two variations of immersion suits throughout the ship. The SOLAS training manual does not address both styles and creats potential confusion during an emergency.

0950-Electric equipment in general-The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The crew has spliced wires to the bus bar in 440V power panels in two locations, which requires the panel to remain open creating a safety hazard for the crew.
