Vessel reported a leak on the port propulsion shaft seal requiring the inflatable shaft seal to be activated. Vessel authorized to go directly to Marathon to discharge cargo and then shift to inner anchorage so divers can make repairs in accordance with submitted repair proposal and risk assessment.

Vessel has a fracture on a vertical stanchion sole plate weld for on deck catwalk and cargo tank deck plating in vicinity of 4C cargo oil tank, centerline frame 80. Vessel is prohibited from loading cargo until permanent repairs are conducted in accordance with class approved repair proposal in Port Angeles, Washington.

Faults with the vessel's engineering alarm system was noted during the USCG's annual ACP oversight inspection. The following alarms were noted: Engine room FIP A network fault, DAU6S/DG2s rack 1 slot 2 I/O fault, HMI server changeover BPNWS1, DAU5S/DG2s rack 1 slot 9 I/O fault. Make repairs to the alarm system to remedy noted faults and prove proper operation through complete ship board testing of the vessel's alarm DVTP dated 14-OCT-2016 and PSTP bearing ABS' approval stamp dated 03-MAY-2010 witnessed by the ACP Class surveyor.
