If natural ventilation is provided for in a space protected by a clean agent extinguishing system, the ventilation must be capable of being easily and effectively closed off. Multiple vent louvers on ventilation to engine room unable to close or be secured. Repair louvers and provide means to secure vents closed.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Found an approximately three (03) inch in diameter wastage/hole in aft deck, creating down flooding hazard into the lazarette. Submit repair plan for MI approval.

Each vessel must—(1) Carry not less than 12 rocket parachute flares approved under approval series 160.136. Flares onboard are not the approved rocket parachute flares. Replace flares with rocket parachute flares.

The minimum luminous intensity of lights shall be calculated by using the formula (Table 84.14(b)). Stern light luminous range calculated to be substandard. Vessel restricted to daytime operations until light is replaced.

No more than 19 liters (five gallons) of flammable liquids may be stowed in any machinery space. The flammable liquids must be in containers of 3.8 liters (one gallon) or less. Vessel found with multiple hydraulic and lube oil containers larger than 5 gallons in the Engine room. Remove excessive combustible liquids.

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended and so that they can be kept in a clean, workable, and sanitary condition. Galley fridge not secured to deck or bulkhead creating a potential safety hazard to crew. Properly secure fridge to deck or bulkhead.

Electric installations on vessels must ensure, protection of passengers, crew, other persons, and the vessel from electrical hazards. Outlet on center support in engine room found without a cover. Install cover.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. Dead end cables identified under operating station. Remove or secure dead end cables.

With the exception stated in paragraph (a)(2) of this section, it shall be the responsibility of the master to assure himself that all exposed cargo hatches and other openings in the hull of his vessel are closed, made properly watertight by the use of tarpaulins, gaskets or similar devices, and in all respects properly secured for sea before leaving protected waters. Port cover for #2 void found damaged and unable to properly close. Repair or replace cover.

A pipe joint or a hose connection fitting that has a gasket must be electrically bonded. Port and Stbd fuel tank pipes on top of the tanks found with flexible hose in line with no bonding. Provide bonding to fuel tank piping.

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. EPIRB registered to wrong call sign. Update EPIRB registration.

You must contact the cognizant Officer in Charge, Marine Inspection to schedule an inspection at a time and place which he or she approves. No written application is required. MI unable to complete annual inspection. Vessel must clear the three (03) action code 60c and the one (01) action code 17c prior rescheduling annual inspection and the carriage of passengers/cargo.

Attachments and fittings. Lifejackets must have the following attachments and fittings:(i) Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. Life jacket lights were either inoperable or had expired batteries.

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The height of rails or bulwarks shall be at least 39.5 inches from the deck except that where this height would interfere with the normal operation of the vessel, a lesser height may be approved by the Commandant. At exposed peripheries of the freeboard and superstructure decks, the rails shall be in at least three courses, including the top. The opening below the lowest course shall not be more than 9 inches. The courses shall not be more than 15 inches apart. Section of railing in aft port quarter, aft midship are broken and or missing courses. Also, hand rail on wheelhouse roof fractured at base on afterside.

The stairways, corridors, and doors shall be so arranged as to permit a ready and direct access to the various lifeboat embarkation areas. Port side railing is bent inward in way of port side main cabin emergency exit door. This, combined with the bent outside door lever, restricts the opening to just 14 inches.

During the term of the vessel's certificate of inspection, the vessel must be in compliance with its conditions. Deficiencies 2, 3, 5, 6, 7, and 9 provide objective evidence that the vessel is substandard and not pro-actively managing technical and operational requirements. Vessel must complete a satisfactory COI re-inspection prior to any movement.

Each arrangement for the storage, distribution, and use of oil in a pressure-lubrication system must as well as comply with § 56.50–80, be such as to ensure the safety of the vessel and all persons aboard. All 04 fuel filter housings supplying fuel to both MDE's are leaking fuel with fuel accumulating on the deck plates and bilge.

Every crewmember with emergency duties assigned on the muster list must be familiar with their assigned duties before the voyage begins. Crewmembers were unfamiliar with their duties during emergency drills.

For each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. The indicator must show whether the door is open or closed. Indicator light in the pilot house does not recognize if the watertight door is open or closed.

Each inflatable lifesaving appliance and marine evacuation system must be serviced within 12 months of each subsequent servicing. Liferaft annual service lapsed on 12OCT2023.

Approved portable fire extinguishers and semi-portable fire extinguishing systems must be installed in accordance with Table 95.50–10(a) of this section. The location of the equipment must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguishers are not installed with the required brackets identified on the fire extinguisher.

Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. MI observed no entries in log book indicating drills are being conducted.

Each panelboard must have a circuit directory cardholder that has the circuit designation of each circuit. Multiple breakers in the engine room 110v panel do not have labels identifying the equipment being served.

The following types of retro-reflective material are approved under this specification: Type II—Weather resistant material used on continuously exposed rigid surfaces. MI observed 02 life ring buoys with weather-deteriorated type II retro-reflective tape.

The registers used in connection with cargo gear certification must have all required entries fully completed as of the dates indicated, shall be kept current, and shall include the following: Register of the cargo handling machinery and the gear accessory thereto, certification of the testing and examination of winches, derrecks, and their accessory gear; certification of the wire rope. Master provided 2017 Crane certificate (CAL-OSHA) indication quadriennial test due OCT (correction AUG 1) 2021, with annual checks between. No record of Quadriennial test in 2021 provided. Annual tests through 2019 provided, and wire rope and socket data provided. Quadriennial test data sticker provided for OCT 11, 2022, but not information as to the weight tests completed. (705c, prior to operation of crane)

Fire hazards to be minimized. Hydraulic oil on lazarette deck plates and in engine room.

Multiple cables should be supported in metal hangers or trays, arranged as far as practicable. Mettalic band stripping used for all cable support should be steel and corrosion treated if not in corrosion resistant material. The support for all cables should be such to prevent undue sag, but in no case should exceed the distance between frames or 610 mm, whichever is less. Wiring cable above the port engine was supported by plastic cables.

Each survival craft must be stowed in a secure and sheltered position...six-man life raft found loose with securing line able to slip off raft.

Medical chest and first aid kit found expired.

Your vessel may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Your Certificate of Inspection, valid for 5 years from 15JUN2016 - 15JUN2021, has expired. Vessel must complete a satisfactory COI Inspection, prior to departure.

YOUR VESSEL MUST UNDERGO AN ANNUAL INSPECTION WITHIN THE 3 MONTHS BEFORE OR AFTER EACH ANNIVERSARY DATE. VESSEL IS OVERDUE FOR AN ANNUAL INSPECTION. VESSEL MUST COMPLETE AN ANNUAL INSPECTION PRIOR TO CONDUCTING CARGO.

It shall be the duty of the Master or other Person in Charge of the vessel to see that all provisions of the COI are strictly adhered to. Vessel failed periodic COI inspection. 46 CFR 97.50-1 (a)

Periodic inspection. Your vessel must undergo a periodic inspection within 3 months before or after the second or third anniversary of the date of your vessel's Certificate of Inspection. This periodic inspection will take the place of an annual inspection. Vessel shall be a Periodic Inspection. 46 CFR 91-27.1 (b)

Distress signals. Each vessel must carry not less than 12 rocket parachute flares approved under approval series 160.136; and stow the flares on or near the vessel's navigating bridge. Vessel only had 04 hand night flares and 3 day smokes signals onboard. 46 CFR 199.60 (c)

Shutoff valves. Shutoff valves shall be installed in the fuel supply lines, one as close to each tank as practicable, and one as close to each fuel pump as practicable. Remote fuel shutdown for FWD starboard fuel tank was inoperable. 46 CFR 56.50-75 (b) (4)

The actions and observations noted in this section shall be entered in the official log book. This section contains no requirements which are not made in other portions of this subchapter, the items being merely grouped together for convenience. (a) Onboard training, musters, and drills: held in accordance with subchapter W (Lifesaving appliances and Arrangements) of this chapter. There no records of current drills and training being conducted onboard. 46 CFR 97.35-5

All other openings to a protected space must be capable of being closed. Doors, shutters, or dampers must be installed for openings in the lower portion of the space. Openings in the upper portion of the space must be capable of being closed off either by permanently installed means or by the use of canvas or other material normally carried on the vessel. Cable penetrations from the battery spaces to the engine room were found to be completely unsealed. 46 CFR 95.16-30 (c)

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, for safe and efficient operation of the vessel. Bilge suction lines STBD FWD fuel tank void were found to be leaking from flexible joints. 46 CFR 56.01-1 (b)

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. Cable penetrations on FWD bulkhead of battery room are unsealed and not weathertight. 46 CFR 91.25-50 (a)

Flexible hose connections should maintain metallic contact between the sections of the fuel-supply lines; however, if they do not, the fuel tank must be grounded. FWD fuel tanks have flexible hoses installed and do not have bonding between filling and tank. 46 CFR 56.50-70 (b) (2)

Provide LR/IBA prior to operating beyound a LBS route.

Repair exhaust leaks at turbos on both main diesel engines.

Make vessel available for dry-dock examination IAW 46 CFR 94.40. Item to be completed NLT 5 FEB 2009, the satisfaction of an attending USCG marine inspector, and prior to operating.

Repair/replace lapped ("slugged") section of transverse frame in void 4 at centerline (keelson).

Gauge & re-weld failed welds in void 3 at centerline (keelson) & void 4 (longitudinal).

Use or remove dead-end cable in way of lazarette-engine room bulkhead; seal bulkhead penetration.

Make temporary repairs to the bow ramp as per submitted plans.

Submit plans to the local OCMI, for approval, to make permanent repairs/replace the bow ramp.
