The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons onboard. PSCO's witnessed a significant leak on boiler No. 1 steamline. 74 SOLAS (04) II-1/26.1

EVERY TANKER SHALL BE PROVIDED WITH MEANS TO ENABLE CREW TO GAIN SAFE ACCESS TO THE BOW. PSCO OBSERVED CARGO DECK CATWALK DECK GRATING USED FOR ESCAPE OR ACCESS TO FIRE FIGHTING EQUIPMENT WAS MADE OF A COMPOSITE MATERIAL. VESSEL DID NOT PROVIDE PROOF THAT THE MATERIAL USED MEETS FIRE INTEGRITY STANDARDS FOR ITS INTENDED SERVICE. 74SOLAS(04) II-1/3-3.2 MSC.62(67)

The navigation equipment required by 33 CFR 164 shall be maintained in operable condition. Vessel master reported the x-band radar is inoperative.

Arrangements for oil fuel, In a ship in which fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the asfety of the ship and persons on board and shall at least comply with the following provisions. PSCO observed oil fuel leaks on number one, two, and three ships service generators. (74 SOLAS (04), II-2/4.2.2) (17ac)

The Company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be stablished by the company. PSCO observed vessel overdue on maintenance hours for number two and three ships service generator fuel pumps. ISM audit to be conducted with three months based on these findings and previous Coast Guard findings. (74 SOLAS IX/3.1, ISM CODE 10) (ac)

Arrangements for oil fuel, In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board and shall at least comply with the following provisions. PSCO observed number three ships service generator primary fuel filter manifold, draining fuel from a housing leak indicator port. The leak was continual and directed to a bucket via flex hose. 74 SOLAS (04) II-2/4.2.2

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter diameter and is at least 25.4 millimeters long. It must be located at the top of the flange face3, midway in line with the bolthole pattern. PSCO observed on all 4 VCS lines, the permanent stud is missing. Vessel is prohibited from transferring cargo requiring the use of VECS. 46CFR39.2001(j)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed two hydrostatic relases for liftrafts are expired. 74 SOLAS (2014 CONS) III/20.2 17 A

Each MSD must be free of design defects that would allow toxic substances to escape to the interior of the vessel. Vessel's MSD had opening on top which did not match the approved drawing. [33 CFR 159.95(a)(1)]

The vessel experienced a steering irregularity while in the Mississippi River.

Each transfer pipe system must have a static liquid pressure test at 1.5 times the maximum allowable working pressure (MAWP) annually or as part of the biennial and mid period inspections. Vessel has not had a static liquid pressure test at 1.5 times the MAWP in the past 12months. 33CFR156.170 70a; Recitify deficiencies prior to next US port after sailing foreign to the satisfaction of Class.

Vessel shall be fitted with an oil discharge monitoring equipement (ODME) that shall automatically stop the discharge of oily mixture into the sea that exceeds 30 liters per nautical mile. Vessel could not demostrate proper operations of the ODME. Vessel may undertake one ballast voyage before proceeding to a repair port and must demonstrate proper ODME operations prior to cargo operations. 15a.

Aft foam monitor on cat walk did not operate when tested. The remaining 05 monitors were tested and operated properly. Rectify prior to deptaure to satisfaction of CG. Violation of SOLAS 74/78 2004 Cons. Ed. Chapter II-2/10.8
