Each survival craft required on a vessel by this part must meet one of the following: For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. Buoyant apparatus(BA) may be used to meet requirements for life floats(LF) if the buoyant apparatus was installed on board the vessel on or before March 11, 1996, and if the buoyant apparatus remains in good and serviceable condition. Vessel 6 has buoyant apparatus manufactured in 2021 on board. Vessel is to replace one 22 person BA per year with an approved 22 person LF.

Whenever damage or deterioration to hull plating or structural members that may affect seaworthiness of a vessel is discovered or suspected the cognizant OCMI... May require the vessel to be drydocked or taken out of service to assess the extent of damage. Prior to returning the vessel to service, CG MI's should inspect The following items: Per Email sent from USCG MSU Toledo on 19SEP24: 1. Have shafts and props ready for CG inspection prior to installation. 2. Have Port side forward most Transducer ready for CG inspection. 3. Have water Lubricated strut Bearings ready for CG inspection. 4. Have "A-Frames" ready for inspection prior to installation. 5. Have vessel available and ready for inspection after the above items are verified by CG MI's and vessel is back in the water ready to come back into service. 46 CFR 176.600 (d) 46 CFR 176.670

The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is to be hauled out or placed in a drydock or slipway in compliance with § 176.600 or to undergo repairs or alterations affecting the safety of the vessel, together with the nature of any repairs or alterations contemplated. Hull repairs or alternations that affect the safety of the vessel include but are not limited to the replacement, repair, or refastening of planking, plating, or structural members including the repair of cracks. Vessel had reported damage to keel, but did not notify CG for attendance of repairs or submit a repair plan prior to undertaking repair work.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel to make permanent repairs to hull following damage to stern/Keel area prior to the carriage of passengers. Repairs to be approved by cognizant OCMI.

A HATCH COVER, EXPOSED TO THE WEATHER MUST BE WATERTIGHT AND BE ATTACHED TO THE HATCH FRAME BY HINGES, CAPTIVE CHAINS, OR OTHER DEVICES OF SUBSTANTIVE STRENGTH TO PREVENT ITS LOSS. MULTIPLE HATCHES ON THE EXPOSED MAIN DECK HAD WASTED/MISSING CAPTIVE CHAINS. 46CFR179.360(b)(2)

ON A VESSEL IN WHICH THE NUMBER OF PASSENGERS ALLOWED ON AN UPPER DECK IS LIMITED, THE MAXIMUM NUMBER OF PASSENGERS ALLOWED ON THE UPPER DECK SHALL BE MARKED WITH LETTERS/NUMBERS OF AT LEAST 1-INCH. THE MAXIMUM NUMBER OF UPPER DECK PASSENGERS WAS NOT MARKED. 46CFR185.602(g)

EACH VESSEL SHALL CARRY ON BOARD A FIRST AID KIT APPROVED IN ACCORDANCE WITH 160.041 OF THIS PART. THE VESSEL HAD A FIRST AID KIT THAT DID NOT MEET THE REQUIREMENTS OF 160.041. 46CFR184.710

RADIO STATIONS IN MARITIME SERVICE MUST BE LICENSED BY THE FCC. THE VESSEL DID NOT HAVE A CURRENT SHIP STATION AUTHORIZATION. 47CFR80.13(a)

EPRIB battery expired. O/O was proactive and has a new EPRIB on order. Restricted their route to less than 3 miles.

Hydrostatic release not marked with expiration date.

Numerous pendants and life lines worn and cracked on all 4 LF.

Painter Line on 2 LF were not properly secured to vessel.

Fuel shutoff valves not accessible during an emergency situation.

Flexible hose does not have required hose clamps on SSDG and MDE systems.

FP not operational during inspection because hose burst.

Provide training on FF systems onboard for crew.

Each lifefloat must be fitted with pendants. Replacemnt pendants must meet requirments in 46 CFR 160.010. Starboard side 22 person life float needs to be provided with pendants. Line is broken and frayed. Vessel restricted to 52 persons on Limited Coastwise voyages beyond three miles from shore.

The number of persons capacity must be marked in clearly legible numbers on each life float. Mark lifefloats with person capacity (2@15,and 2@22).

The number and identification of items stowed inside must be marked clearly on each container of lifejackets. Properly label stowage number for child PFD's in main cabin (11).

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. Mark paddles for all life floats with vessel name. (previous marking is worn off)

Each lifefloat must be fitted with a light. Replace battery in LF Waterlight so that the vessel has an approved light for each Life Float.

Provide new hydro and mark installation date for the EPIRB.

Replace float light line on life float.

Secure holes in FWD passenger cabin deck. Pipe penetration and wire penetration STBD and PS

Replace safety pin on manual pull at operating station.

Provide aspirin and eye wash for the first aid kit.

Provide new batteries for four float lights.

Replace Retro tape on PFD's as needed.

Replace hydro release on EPIRB

Provide passenger safety orientation.

EPIRB registration was expired

Provide 3 orange smoke and 3 red hand held flares

All paddles missing from lifefloats
