The arrangements for the storage, distribution and utilization of oil in pressure lubrication system shall be such as to ensure the safety of the ship and persons onboard. PSCO observed excessive L/O leaks from the No: 3 and 4 air compressor motors.

In considering the design of such equipment and approvals, the administration shall have regard to the specification recommended by the organization. MEPC.107(49)/1.1.5 requires ship. Staff training should include familiarization in the operation and maintenance of the equipment.

The company should establish procedures, plans and instructions, including checklists as appropriate. PSCO's identified multiple areas of oil accumulation around the engine room. Standing oil was observed on main engine and in the purifier room. Several plastic buckets cut to fit under machinery are in use to collect dripping oil, and several metal deflecting plates are in place to deflect spraying oil. These deflection plates are located on the main engine and in the purifier space. Oil-soaked lagging was also discovered in these areas. After reviewing the vessel's "[name withheld]", Line item 6 refers to addressing excessive oil accumulation and line item 11 refers to keeping the engine room in an excellent condition. Additionally, the injectors and fuel pumps have exceeded their maximum running hours per the vessels SMS: fuel injectors to cylinders 1, 4 and 8; fuel pump to cylinder 1. An external SMS audit is required.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the relevant regulations. PSCO’s observed significant leakage of the jacket water main engine cylinder number 5.

Every steam pipe and every fitting connected there to … shall be so designed, constructed and installed as to withstand the maximum working stress … PSCO’s observed active steam leak from piping in vicinity od diesel engines number 3 and 4.

Doors required to be self-closing shall not be fitted with hold back hooks. Both fire screen doors to purifier space are identified as self-closing on the FCP. Both were found tied open. Purifier space was found to have excessive leaks and accumulation of flammable liquids.

Oil fuel pipes … shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside of the space concerned. Valve from HFO tank to boiler found to have its quick closing valve disconnected. Crew states that the tank is no longer in service.

EACH EMERGENCY GENERATING SET ARRANGED TO BE AUTOMATICALLY STARTED SHALL BE EQUIPPED WITH STARTING DEVICES APPROVED BY THE ADMINISTRATION WITH A STORED ENERGY CAPABILITY OF AT LEAST THREE CONSECUTIVE STARTS. A SECOND SOURCE OF ENERGY SHALL BE PROVIDED FOR AN ADDITIONAL THREE STARTS WITHIN 30 MIN UNLESS MANUAL STARTING CAN BE DEMONSTRATED TO BE EFFECTIVE. PSCO OBSERVED EMERGENCY GENERATOR WAS UNABLE TO START BY PRIMARY AND SECONDARY MEANS.

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. PSCO observed the natural ventilation closure on bay 22 starboard side is bent and will not close properly.

Ships shall be maintained in compliance with the mechanical requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulation XI-1/1, or with applicable national standards of the Administration which provide an equivalent level of safety. PSCO observed oil leak originating from #3 generator casing.
