The following functional requirements shall be met escape routes shall be maintained in a safe condition, clear of obstacles. Automatic door on starboard forward to embarkation deck was set local mode and did not open not allowing passengers to reach the lifeboats. 74 SOLAS 20 II-2/13.1.2 17ac

Resolved 11 May 2026
Resolution: Crew electrician placed door in automatic mode to ensure safe evacuation through automatic door. Stated that it was place in local to be cleaned so it would open. Discuss with housekeeping procedures to ensure it gets changed back to automatic mode.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The helm steering did not work on Lifeboat 11. The hydraulic steering ram was leaking hydraulic oil. 74 SOLAS 20 III/20.2 17ac

Resolution: Crew replaced hydraulic ram seals and conducted steering test under USCG supervision to ensure fully functionalilty and effectiveness of repair.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCOs observed previous nonconformity identified on port scrubber overboard discharge connection with excessive leak. 74 SOLAS 20 III/13.1.1

Each survival craft shall be stowed so that neither the survival craft nor its storage arrangements will interfere with the operation of any other survival craft or rescue boat at any other launching station. PSCOs observed the forward davit launched liferaft stations were obstructed by the support bracket, catching the release hook on the liferaft davit hook which prematurely released the liferaft. 74 SOLAS 20 III 13.1.1

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40s and no less than 10s from the beginning of their movement. PSCOs observed fire doors 1.3.03, 1.3.28, 1.5.02, 2.5.01, and 4.8.10 did not meet closure time requirements. 74 SOLAS 20 II-2/9.4.1.1.5.2

On 16APR21 it was discovered that the vessel arrived into Port of Miami on 08APR21, but had failed to submit a NOA -33CFR160.212(a)(4)(ii)

All survival craft required to provide for the abandonment by the total number of persons onboard shall be capable of being launched with their full complement of persons and equipment within a period of 30 minutes from the time the abandon ship signal is given. The crew was unable to bowse the aft station life raft for 7 minutes. The life raft launching station serves 4 life rafts. The second deployment of the forward station was 3 minutes and 20 seconds. 74 SOLAS (04) III/ Regulation 21 Chapter 1.1.4.
