External high pressure fuel delivery lines between the high pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high pressure line failure...the jacketed piping system shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. PSCO observed thermal protection on double jacketed lines (M/E, cylinder 1-10) not secured (missing bolts to each cylinder) as designed. Fuel oil was observed pooling at the base of cylinders (#1-10). PSCO also observed missing excess fuel drip pipe on # 1 cylinder. 17-a c, 74SOLAS(14), CH II-2, Reg 4.2.2.5.2

PSCO observed the port anchor chain link stud broken free.
