Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at a constant marine load. Furthermore, repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Proposed alterations must be approved by the OCMI before work is started. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Each entrance to a space storing Carbon Dioxide cylinders, a space protected by Carbon Dioxide system must have proper marking,. The space where Carbon Dioxide is stored, protected and might migrate is missing proper marking.

Dampers - provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge opening for a space protected by a fixed gas extinguishing system. Dampers are broken and do not shut.

Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. Gauge glass was made of a non-heat resistant material. 46 cfr 182.440 (a)(v)(7)

46 CFR 181.410(b)(10) - A device must be provided to automatically shutdown power ventilation servicing the protected space and engines that draw intake air from the protected space prior to release of extinguishing agent. No auto shutdown arrangement in place. Install automatic ventilation shutdown pressure switch.

46 CFR 176.830(a) - Protective device must be in satisfactory condition. Bilge pumps adjacent t o stuffing boxes not hard wired and bundled with dead-ends. Install junction boxes and secure wires.

Prove proper operation of installed fire pump.

Prove proper operation of installed bilge system.

Repair/Replace all life floats to be in good working order. If purchasing new life floats, notify USCG prior to replacement.
