MI shall make tests and inspections deemed necessary to assure the safety and seaworthiness of a vessel. An exhaust leak was found on the bow thruster exhaust system/ piping within the main machinery space. 46 CFR 126.100

Each OSV must have a line-throwing appliance. Vessel has shoulder gun type line-throwing appliance (approval 160.031), was missing the 500ft. auxiliary line. 600ft. of auxiliary line was acquired and brought on board.

A rescue boat is not required for a vessel operating on the continental shelf of the United State if the OCMI determines the vessel is arranged to allow a helpless person to be recovered from the water. OCMI determination is that the platform landing shall be a minimum of 36 x 36 inches. The rescue platform on board was 36 x 24 inches. - 46 CFR 133.135(c)

The circuit breaker of a generator to be operated in parallel with another generator must have reverse-power or reverse-current trips. The reverse power relays were unable to be exercised during the inspection. A third-party technician attended, finding relays on both the forward and aft generators bad. The bad components were replaced, with proper operation being verified by the attending technician. - 46 CFR 129.323(b) // 46 CFR 111.12-11(f)

The settings of safety relief valves on pressure vessels must be inspected twice every 5 years and no more than 3 years may elapse between inspections. Bench test or replace PV valves.

Each generator arranged for parallel operation must have reverse-power trips. Conduct test to verify activiation of such trips.

No OSV that carries flammable or combustible liquid with a flash point of below 140 °F on deck or in integral tanks, or is involved in servicing wells, may have electrical equipment installed within 10ft. of a source of vapor on a weather deck unless the equipment is explosion-proof or intrinsically safe under 111.105-11 of this chapter. Non-intrinsically safe equipment and potential ignition sources were noted within hazardous zones and their outlying areas to include: Portable radios, prime movers w/ PTO, diesel powered air compressors, aft deck house lighting, and main engine exhaust. - 46 CFR Part 129.520 / 46 CFR Part 111.105-11 - Prior to Backflow Operations

Each safety relief valve installed on a cargo tank shall be connected so that it extends to a height above the weather deck equal to at least 1/3rd of the beam of the vessel and to a minimum of at least 10ft. and shall terminate at a comparable distance to any source of vapor ignition. Vessel had open hatch for gas buster during transfer operations within a hazardous area. -46 CFR Part 30.01-5 / 46 CFR Part 38.20-1

Each applicant for approval of plans and for an original Certificate of Inspection shall submit the following: General Arrangement Plans. The vessel was not operating IAW it’s General Arrangement Plan. The port aft life buoy was found missing from its designated location. Per crew, its bracket was broken, and the buoy stored elsewhere for a month. - 46 CFR Part 127.110

Twenty-five cartriages shall be provided for shoulder line throwing gun. Only had twenty-two cartriages available. 46 CFR 160.031-4(d) 46 CFR 133.170(a) and (c)

Keel cooler machinery inlet valve for bow thruster prime mover is broken. Replace appropriately.

High bilge alarm must be visual and audible at the pilot house. None of the vessel's bilge alarms were visual and audible at the pilot house. 46 CFR 130.460

The marine inspector shall determine whether the vessel is in a safe condition, including fire hazards. Accumulation of oil in the engine room bilge is too excessive and poses a fire hazard. 46 CFR 126.430 (m)

Instructions, including diagrams, for a shift of steering gear and for a shift to the alternative steering must be on water-resistant material and posted at each steering station and relating in order, the different steps to take. Auxiliary steering in engine room did not have any instructions for transfer from normal/remote to auxiliary/local. 46 CFR 131.845(a)

Pressure indicating devices shall be not less than 1.2 and not more than 2 times the relieving device set pressure. Pressure gauge maximum range on aft port and starboard air receivers were not set high enough. 46 CFR 54.15

Two (02) watertight doors leading from mess deck into bow thruster compartment and from bow thruster compartment into engine room were not watertight. IAW 46 CFR 131.520, hatches and other openings should be made properly watertight by the use of gaskets or similar devices.

Forward refrigeration condensing unit had a corroded electrical installation box for its power/controlling unit. IAW 46 CFR 126.510 (a)(3), repairs must be made due to lack of vessel maintenance and safety concerns.

Two (02) wasted diesel fuel return hoses from Port and Starboard Main Diesel Engines. Two (02) incorrect fuel hoses on both forward and aft generators. IAW 46 CFR 56.60-25 (b) (1), nonmetallic flexible hoses much be IAW SAE J1942 and must be in good condition for service.

Starboard air receiver relief valve was missing servicing tag and proof of set pressure. IAW 46 CFR 54.15-10(a), relief valves shall be set to relieve at a pressure not to exceed the maximum allowable working pressure of the pressure vessel.

Fire detecting equipment must be available and functional inspection; smoke detector in main deck accommodation space was not in working condition during inspection.

Remote stopping-systems must be clearly marked to show what system each controls; remote stops on back deck deck were insufficiently labelled to indicate system control.

Each watertight hatch must be marked on both sides in letters at least 2 inches high that state the followingL “WATERTIGHT HATCH - KEEP CLOSED WHEN NOT IN USE.” Hatch on back deck was insufficiently labelled to meet requirement.

Means of escape must be provided for the egress of a space normally accessed by crew; latch to freezer required adjustment such that the door could be opened from the inside should personnel be shut in in order to escape the area.

Vessel did not have auxillary line onboard for line throwing gun. Provide auxillary line.

Vessel's chart has not been corrected since printing in 2017. Provide updated chart and maintain charts with correction kit.

Port MDE had an exhaust leak. Repair exhaust leak.
