0720-Fire Fighting Equipment- Machinery spaces of category A and above in the case of periodically unattended machonery space. The fire extinguishing system shall have both automatic and manual release capabilities. PSCO discovered that the hyper mist system in the machinery space was placed on manual mode not in automatic mode as required. Continued in Narrative.

PSCO discovered that the smoke detection system in the cargo spaces were rendered non operational. The fuse to the system was intentionally removed. 74 SOLAS (14) II-2/19.3.3-FSS Code 10.2.1.2.

03-2500- ISM related deficiency-Objective evidence discovered during an expanded ISM exam revealed the vessel was not following proper shipboard operations as they applied to their fire detection and fire suppresion systems. PSCO's confirmed the crew shifted the hyper mist system in the machinary space to manual mode and removed fuses for the fire detection system. Continued in Narrative.
