Electrical installations should be such as to minimize the risk of fire and explosion from flammable products. 14 explosion proof light fixtures were found with serious cracks in the lens, compromising the ex rating. Crew shall replace with on-hand spares. lights in compressor room and airlock must be replaced prior to cargo. The other 9 must be replaced prior to departure.

PSCO believes the nozzles installed around the vessels booster pumps to be too small to provide adequate coverage regardless of the quality of the water supply.

Every company should develop, implement, and maintain a Safety Management System which includes instructions and procedures to ensure safe operations of ships. While observing testing of the inert gas (IG) system, PSCO's personal four-gas meter alarmed for high levels of carbon monoxide. The condensate drain line was found with the valve in the open position allowing IG to freely leak into the engine room space. Shipboard procedures state that the condensate drains should be checked and closed if necessary before starting the IG system.
