Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel has a fuel leak coming from starboard fuel pump lines on the MDE that are leaking on to governor.

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. The starboard engine has an exhaust leak that appears to be coming from the seal of the turbo connecting to the block.

Each fixed power bilge pump must be self priming. It may be driven off the main engine or other source of power. It must be permanently connected to the bilge manifold and may also be connected to the fire main. If of sufficient capacity, a power bilge pump may also serve as a fire pump. Fixed bilge pump was unable to provide satisfactory operation. 46 CFR 182.520(c)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Port MDE propulsion shaft is exposed. 46 CFR 177.960

Protection from mechanical damage must be specifically considered and all protective covering or shields must be installed to the satisfaction of the cognizant [name withheld] flexible fuel oil lines are not protected from chafing. 46 CFR 182.720 (b)

Doors on a cooking appliance must be provided with hinges and locking devices to prevent accidental opening in heavy seas. Oven door is missing hinge and locking device. 46 CFR 184.220 (a)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. The following restrictions apply:(1) Flexible nonmetallic hose must be complete with factory-assembled end fittings requiring no further adjustment of the fittings on the hose, or field attachable type fittings may be used. Hose end fittings must comply with SAE J-1475 (incorporated by reference; see 46 CFR 175.600). Generator fuel hoses are hose clamped. 46 CFR 182.720(e)(1)

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Generator fuel filter is missing drip pan(bowl). 46 CFR 182.455(b)(6)

A lighting fixture must be installed as follows:(1) Each fixture must comply with § 183.200. Engine room light fixtures were found to be improperly installed. 46 CFR 183.410(c)(1)

When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. The flowmeter sensor on the cooling water pump for the starboard main diesel engine was disconnected. 46 CFR 182.420

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided in compliance with § 177.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. The lagging on the vessel's port main diesel engine was damaged. 46 CFR 182.430 46 CFR 177.970

Temperature ratings of electrical equipment must meet the requirements of 46 CFR 111.01-15. A 50 °C (122 °F) ambient temperature is assumed for all rotating electrical machinery in engine rooms, unless it can be shown that a 45 °C (113 °F) ambient temperature will not be exceeded in these spaces. If electrical equipment is utilized in a space in which the equipment's rated ambient temperature is below the assumed ambient temperature of the space, its load must be derated. MI observed 40 degrees Celsius rating for the vessel's generator located in the engine room with no supporting documentation that either the space was cleared for 40 degrees Celsius or that the generator had been derated. Instructed owner/operator to provide objective evidence. 46 CFR 183.230 46 CFR 111.01-15

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date Coast Guard Light List. The vessel did not have a 2023 Coast Guard Light List on board. 46 CFR 184.420(a)(3)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. The portable light for the machinery space is missing. 46 CFR 183.430

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Two of the three buckets were not labeled "FIRE BUCKET" in a contrasting color. 46 CFR 181.610

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Found 5 life jackets with expired lights. Vessel representative replaced expired lights with new lights that have valid dates. 46 CFR 180.75(a)

Vessels operated beyond the 20NM limiitation but not more than 100NM from the nearest land, must be equipped with a MF-DSC frequency transmitter. Vessel has SAT Phone marked on FCC Safety Cert in place of MF-DSC but did not have SAT phone on board. Vessel's route is restricted to Coastwise Route. 47CFR80.905(a)(2)

The installation and location of portable fire extinguishers must be to the satisfaction of the [name withheld] galley fire extinguisher was placed on the ground and not secured in manufacturers bracket.46CFR181.520

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Vessel did not have operator permit for licensed captain on board. 47CFR80.159(e)(1)

Except on a vessel limited to daytime operations, the compass must be illuminated. Vessel's compass light is not working properly. 46CFR184.402(c)

The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. These locations must be readily accessible to the intended reader and may include embarkation points, food service facilities, garbage handling spaces, living spaces, and common areas on deck. Vessel does not have a garbage placard posted. 33CFR151.59

A manned oceangoing ship of 40FT or more in length that is documented under the laws of the US engaged in commerce or has birthing or a galley shall ensure that the ship is not operated unless a garbage management plan is on the ship and that each person handling garbage follows the plan. Vessel does not have a garbage plan on board.

Repairs to the hull that affect the safety of vsl must not be made without OCMI approval. Through-hull penetration for AC line under waterline is leaking. Vessel must repair.

Vessels with pre-engineered fixed fire systems must have means to reset devices used to automatically shut down ventilation systems and engines. Vessels reset device was located in the engine room.

When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. Vessel did not have warning device.

A vessel of at least 26 feet in length must be provided with individual bilge lines and bilge suctions for each watertight compartment. The bilge line in the passenger compartment was not installed properly.

Each hatch exposed to the weather must be watertight. The fwd and aft hatches have wasted gaskets.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with §161.012 in subchapter Q of this chapter. One lifejacket had expired light.

Piping systems used in vital systems must be made of ferrous material except when nonmetallic piping materials are permitted by §182.720. Each Bilge line has flex hoses throughout.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck, or, if it is readily accessible under service conditions. The screw down valve was not operable.

Rigid nonmetallic hoses must be adequate for its intended service. Port exhaust cooling hose was crimped and starting to crack to due to torsion. 46 CFR 182.720(d)(4)

Vessel must carry up to date charts. Vessel charts were out of date. 46 CFR 184.420(a)(1)

Vsl shall have means to obtain evidence of alcohol in the event of a marine casualty. Vsl alcohol test strips were expired. 46 CFR 185.210

A vessel not required to have fire pump must have 3 buckets with 2.5 gallon capacity. Vessel had only 2 fire buckets. Vessel rep needs to show proof of 3 fire buckets.

Each lifesaving equipment with an expiration date that expired during annual inspection needs to be replaced. 3 waterlights for lifesaving equipment were expired. Vessel rep needs to replace all expired batteries of all livesaving equipment.

Replace fuel hose on generator.

Replace hose on stbd engine.

Replace fuel hose on port engine.

Repair cracks in hull located in the rudder room.

Repair pin holes in engine room sideshell under the generator exhaust.

Replace hose on fire extinguisher in accomodation space, house worn.
