All engines in lifeboats and rescue boats shall be run for a total period of not less than 3 min...it should be demonstrated that the gearbox and gearbox train are engaging satisfactorily. PSCO observed the rescue boat was unable to engage astern propulsion.

...the essential importance of good housekeeping in the prevention of accidents to personnel...and the avoidance of any conditions on boad likely to be injurious to seafarers' health. PSCO observed significant standing water in the crew laundry space. ILO-147 p44/3(a)

For this purpose, the following functional requirements shall be met: escape routes shall be maintained in a safe condition...PSCO observed that emergency lights were inoperable just aft of the BOSN store, port side between bays 2 and 25, and port side between bays 33 and 35. 74 SOLAS (24) II-2/13.1.2

The purpose of this regulation is to detect a fire in the space of origin and to provide for alarm for safe escape and fire-fighting activity. PSCO observed that the smoke detector in the galley was covered by plastic wrap, impeding its function. 74 SOLAS (24) II-2/7

To prevent the ignition of combustible materials or flammable liquids, the following requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed substantial accumulation due to leaks on both purifiers, poooling underneath the system. Oil soaked rags were found as means of containment. 74SOLAS(20)II-2/4.1.1

ELECTRICAL INSTALLATIONS SHALL BE SUCH THAT THE SAFETY OF PASSENGERS, CREW, AND SHIP FROM ELECTRICAL HAZARDS WILL BE ENSURED. PSCO IDENTIFIED MULTIPLE WIRING ARRANGEMENTS ON STERN WEATHER DECK LIGHTING FIXTURES THAT HAD EXPOSED WIRING WRAPPED AND SECURED WITH TAPE. 74 SOLAS (04) II-1/40.1.3

STAIRWAYS AND LADDERS SHALL BE SO ARRANGED AS TO PROVIDE READY MEANS OF ESCAPE TO THE LIFEBOAT AND LIFERAFT EMBARKATION DECK FROM PASSENGER AND CREW ACCOMODATION SPACES AND FROM SPACES IN WHICH THE CREW IS NORMALLY EMPLOYED, OTHER THAN MACHINERY SPACES. PSCO OBSERVED AN EMERGENCY ESCAPE HATCH ON STARBOARD WEATHER DECK LOCKED WITH A PADLOCK. 74 SOLAS (20) II-2/13.3.1.1

WHERE THE BILGE PUMPS ARE CAPABLE OF BEING STARTED AUTOMATICALLY. MEANS SHALL BE PROVIDED TO INDICATE WHEN THE INFLUX OF LIQUID IS GREATER THAN THE PUMP CAPACITY OR WHEN THE PUMP IS OPERATING MORE FREQUENTLY THAN WOULD BE NORMALLY EXPECTED. IN THESE CASES, SMALLER BILGE WELLS TO COVER A REASONABLE PERIOD OF TIME MAY BE PERMITTED. WHERE AUTOMATICALLY CONTROLLED BILGE PUMPS ARE PROVIDED, SPECIAL ATTENTION SHALL BE GIVEN TO OIL POLLUTION PREVENTION REQUIREMENTS. PSCO OBSERVED NON-FUNCTIONAL BILGE ALARM SENSOR OF AFT BILGE WELL NEAR SHAFT SEAL. 74 SOLAS (20) II-1/48.2

Where the emergency source of electrical power is a generator, it shall be started automatically upon failure of the main source of electrical power supply unless a transitional source of emergency electrical power in accordance with paragraph 3.1.3 is provided; where the emergency generator is automatically started, it shall be automatically connected to the emergency switchboard; those services referred to in paragraph 4 shall then be connected automatically to the emergency generator; and unless a second independent means of starting the emergency generator is provided the single source of stored energy shall be protected to preclude its complete depletion by the automatic starting system. Where a fixed gas fire-extinguishing system is used, openings which may admit air, or allow gas to escape from, a protected space shall be capable of being closed from outside the protected space. PSCOs observed at least 15 cargo hold vent openings on the port and starboard side missing gasket material preventing the openings from being closed to prevent fixed gas fire-extinguishing agent from escaping. 74 SOLAS (2020) II-2/10.4.3 Code 17abc

Every ship shall be provided with a receiver capable of receiving international NAVTEX service broadcasts if the ship is engaged on voyages in any area in which an international NAVTEX service is provide. The vessel could not produce any records of NAVTEX messages being received. Code 50ac

VESSELS OF 10,000 GT OR MORE MUST HAVE, IN ADDITION TO THE RADAR SYSTEM UNDER 33CFR164.35(a), A SECOND RADAR SYSTEM THAT OPERATED INDEPENDENTLY OF THE FIRST. VESSEL S-BAND RADAR IS INOPERATBLE 33CFR164.37(a)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard.
