The annual maintenance and servicing of the fixed-gas fire extinguishing system identified a faulty valve, rendering the system non-operational. The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Provide objective evidence to the CG of satisfactory corrective and preventative maintenance and testing of the fixed-gas fire extinguishing system, prior to the carriage of passengers. 46CFR176.810(b)

Provide 17 adult lifejackets for full complement of persons onboard (152 total). Passenger count reduced to 132 until rectified.

Replace overhead plate IWO air handler space and lazarette. Repair to good metatl. Submit repair plan prior to commencement of repairs.

No Marine Radio Op. Permit onboard, provide proof of permit.

Alcohol test strips are expired, provide proof of obtaining satisfactory test strips.

Provide up to date Compass Deviation Table.

Provide up to date Coast Pilot and Tide Tables.

Replace engine room markings to indicate CO2 protected space; 46 CFR 185.612

Replace expired EPIRB battery, do not transit seaward of demarkation line. 46 CFR 180.64

Renew insulation (crumbled) separating engine room from accommodation and service spaces to meet 46CFR 164.109 if determined by ship builder to be necessary for structural fire protection; 46CFR177.410

Provide evidence degraded engine room insulation is not a respirable health/safety hazard to crew. 46CFR176.830

Aft generator wet exhaust hose is zip tied, make permanent attachment.

Replace broken hose clamp on port engine cooling line.

Replace cooling water hose for aft generator, pinhole leak is in current hose.

Galley is unclean, restore to acceptable cleanliness stardards.

Bilge pump is inoperable, do not carry passengers until bilge pump is operable.

Replace both cutlass bearings on starboard shaft.
