Sea valve for reverse osmosis system failed, flooding the propulsion motor room to approximately 17 inches above bilge well. Sea valve failed at the body and bonnet flange due to wasted and sheered fastener bolts. 74 SOLAS (2020 Cons.) II-1/3-1

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Due to the flood water experienced in the propulsion motor room bilge, the propulsion motor casing experienced an ingress of approximately 1.5 inches in the casing bottom. Provide manufacturer technician report attesting to satisfactory condition of the propulsion motor unit as well as class survey report attesting to satisfactory condition. 74 SOLAS (2020 Cons.)II-1/26

It was reported by vessel's master the failure on radar inter-switch system. Vessel is currently provided with three (3) radars, two (2) short range (x-band) and one (1) long range (s-band). All three radars were provided with three (3) display monitors in the navigation bridge which can be interchangeable to each other, however only two (2) are able to operate at the same time because of the interswitch problem.
