The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel; or the effectiveness or completeness of its lifesaving, fire-fighting, or other equipment. The vessel ran aground and reported damage to both propellers.

Paint lockers and similar compartments must be constructed of steel or be wholly lined with steel. Vessel's paint locker was constructed of aluminum.

Equipment subject to normal oil leakage must be fitted with drip pans or other means to prevent oil draining into the bilge. The drip pans underneath the fuel filters on the vessel's 02 main engines, 02 generators, and bow thruster had a drain which would not prevent oil from dripping into the bilge.

Numerous crushed and fractured structural members present on the port side of the forward hold.

Each OSV must carry at leas one rescue boat... Unless OCMI determines the vessel is arranged to allow helpless person to be recovered from the water. Vessel has rescue platform, Rescue platform is 30"x30". CID memo1 requires platform to be 36"x36".

A class 1 door must be able to withstand a head of water pressure. Portside engine room watertight door had 2 spots on the bottom left and right where the metal was missing. Repair to the satisfaction of attending MI.

Vessel must have navigational lighting in accordance with navigation rules. Improper wiring configuration of port navigation light. Stern light has broken lens.

Embarkation station must be adequately illuminated. Insufficient lighting at embarkation station.

Fire equipment shall be onboard and maintained to satisfy marine portable tank endorsement. Foam eductor not functional. Must be rectified prior to operations relative to portable tank endorsement

Inspection for certification shall include inspections of watertight integrity; cable penetrations piercing watertight bulkhead were not sealed to prevent passage of smoke or flames.

Inspection for certification shall include inspections for fire hazards; hydraulic hose on bow thruster prime mover was chafed; replace prior to use of bow thruster.

Each item of survival equipment with an expiration date must be replaced during the annual inspection if the date has passed; 03 waterlight batteries were expired.

When life floats are stowed in tiers, they must be kept apart by spacers; spacers were missing.

Means of securing doors closed shall include use of gaskets; gaskets for forepeak and engine room escape hatches need to be replaced.

Each vessel must carry up to date paper charts; paper charts were not corrected through the most recent NTM.

Nonmetallic flexible hose must be in accordance with SAE J1942 (incorporated by reference; see 46 CFR 56.01-2) and may be installed only in vital and nonvital fresh and salt water systems, nonvital pneumatic systems, lube oil and fuel systems, and fluid power systems. The outer protective covering on the fuel hose for the port generator was cracked in several places. Vessel shall replace fuel hose on port generator in accordance with SAE J1942 specifications prior to departure. 46 CFR 56.60-25(b)(1)

Means of escape from aft engine room escape scuttle was not operable from inside the vessel. Crew to adjust and teast for ease of operation. 46 CFR 127.240(a).

Vessel is equipped with fixed pitch propellers and is required to have speed and direction indicators for each shaft at the navigating bridge. Shaft tachometers were inoperable duirng propulsion checks. 46 CFR 113.37.5(a)

Vessel must have a means to communicate with the pilothouse from auxiliary means of steering. Vessel's sound powered phone is inoperable.

Weak link loop was placed over painter line holder but no bolt was present. Required capt to secure weak link to vsl.

Lifefloat water light was attached with a light colored synthetic line. Vsl could not produce proof that line was UV resistant. Required vsl to replace line with dark colored synthetic line.

Placards for the general alarm system were not correct. Actuators and audible/visual alarms were labeled "general alarm".

Fuel oil,ballast, and potable water vent check valves contained wasted screens and floats. Required vsl to replace vents as indicated.

Battery boxes in E/R were not secured.

During E/R inspection found bilge containing excessive water. Required crew to remove to holding tank or approved facility.

Crew could not provide rescue harness tending line.

Crew conducted drills but were not proficient. Conducted training and required capt to perform drills and provide proof of satisfactory results.

Mate missing FCC Marine Radio Operators Permit - Vessel required to have a mate with the appropriate permit within the next 30 days

Mount a total of 8 liferings with proper number of lights and line on vsl. Update safety plan to reflect proper location of life rings.
