All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. 2 air receivers in the machinery space did not have the MAWP indicated. 46 CFR 143.300(c)

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Multiple beams, brackets and frames in laz. were wasted away and will require crop and renew at next scheduled drydock to the satisfaction of the attending MI. 46 CFR 140.65(b)

Each distribution panel or switchboard on a towing vessel must be: Totally enclosed and of the dead-front type. Electrical panel near operating station and in machinery space had missing breakers and not covered. 46 CFR 143.560(a)(2)

Vent pipes for integral fuel tanks. Each integral fuel tank must have a vent that connects to the highest point of the tank, discharges on a weather deck through a bend of 180 degrees, and is fitted with a 30-by-30-mesh corrosion-resistant flame screen. Starboard aft fuel tank vent flame screen overly covered with paint and restricting flow of vent. 46 CFR 143.265

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Records were not available for inspection. 46 CFR 140.820

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. All watertight doors onboard were not marked on both sides. 46 CFR 144.160

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: Low fuel level, if fitted with a day tank. Vessel not outfitted with day tank alarm. 46 CFR 143.230

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Pressure relief valves: Twice every 5 years, with no more than 3 years elapsing since last test. Records of tests not available for inspection. 46 CFR 143.245

The inspection will include inspections of the structure, pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire detecting and extinguishing equipment, pilot boarding equipment, and other equipment. Wastage on port side of engine room overheard above port generator exhaust that has epoxy patch. Upper side of patch is in the fidley around a deck drain just before exiting port watertight door. Area to be further inspected by Marine Inspector at next drydock. 46 CFR 136.212

46 CFR 142.315: A towing vessel that is certificated for rivers, lakes, bays, and sounds must be equipped with either an approved 160–B semi-portable fire-extinguishing system to protect the engine room or a fixed fire-extinguishing system installed to protect the engine room. 46 CFR 136.110: Fixed fire-extinguishing system means a carbon dioxide system that meets the requirements of 46 CFR subpart 76.15 and 46 CFR 78.47–9 and 78.47–11, and that is approved by the Commandant. 46 CFR 76.15-35 (c): Enclosure openings. Means shall be provided for closing all openings to the space protected. Vessel has an auxiliary space above engine room that is separated only by deck grating, effectively creating one space from a fixed fire extinguishing perspective. Operator to seal all bulkhead penetrations identified during exam to the satisfaction of the marine inspector. Including above the engine room door and two in the auxiliary space.

46 CFR 78.47-11 (b): Carbon dioxide warning signs. Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces protected by carbon dioxide — “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES DO NOT ENTER UNTIL VENTILATED.” Vessel has an auxiliary space above engine room that is separated only by deck grating, effectively creating one space from a fixed fire extinguishing perspective. Operator to label all doors leading into this space, to the satisfaction of the marine inspector, with the warning as indicated above.

Each towing vessel must have a fire-detection system installed to detect engine room fires. Indicating lights on the fire alarm panel are out. Effect repairs to the satisfaction of the OCMI. 46 CFR 142.330(a)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Emergency lighting outside berthing space is inoperable. Effect repairs to the satisfaction of the OCMI. 46 CFR 143.410(b)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. There are no alarms installed for Low hydraulic steering fluid levels. Install low level hydraulic steering fluid level alarm to the satisfaction of the OCMI. 46 CFR 143.230(a)

The hydraulic steering system must be equipped with fluid pressure gauges at the machinery space. Hydraulic steering system pressure gauge is not installed in the machinery space. Install hydraulic steering fluid pressure gauges. 46 CFR 143.230(c)(5)
