Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas. PSCO found exposed wiring on the #1 and #2 Lube oil purifiers. 74SOLAS(04) II-1/45.5.4

Resolution: Vessel repaired the exposed wire with appropriate sheathing.

Maintenance testing, and inspections shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. PSCO observed crew attempting to use heat gun taped to an iron rod to test a heat detector which does not adhere to the fire detection operating instruction procedures carried on board the vessel. SOLAS 74(am) 2014(cons.) II-2/14.2.2.1

Each vessel of 10,000 gross tons or more must have, in addition to the radar system under 164.35(a), a second marine radar system that operates independently of the first. Vessel reported inoperable S-band radar.

Resolution: Vessel sent in a tech report verifying the radar has been fixed and is now operational.
