Vessel had 3 red hand flares and only 1 hand orange smoke, needs 2 additional hand orange smoke or additional 3 red hand flares to meet requirement.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. Multiple stanchions were loose and/or stripped.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Each life jacket must be marked with Type I retroreflective. Must be marked in clearly legible block capital letters with the vessel's name Adult and child life jackets were stored together. Life jacket storage were not properly labeled. Life jackets need retroreflective and all need to be properly labeled.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States: (a) Every power-driven vessel of 20 meters (65 FT) or over in length while navigating. Vessel was missing Safety Certificate and Bridge-to- Bridge Certificate.

The 2 inch ball valve for the raw water intake for the starboard side MDE would not close properly. Prior to putting the vessel back in the water, replace the frozen valve with a new one and demonstrate proper operation to the satisfaction of an attending Coast Guard Marine Inspector prior to launching. Remove the bung on the port side forward compartment for further inspection of the space. This space must be inspected to the satisfaction of a Coast Guard Marine Inspector prior to July 2, 2022. (Marked #1 on work list)

Remove all lagging/insulation in the overhead of the machinery space to open up all overhead beams for inspection. The lagging/insulation must be removed and the beams and all associated structures must be inspected to the satisfaction of a Coast Guard Marine Inspector prior to July 2, 2022. Properly install all grounding wires located in the Lazarette. The grounding wires must be reattached and adequately provide electrical protection and must be inspected to the satisfaction of a Coast Guard Marine Inspector prior to July 2, 2022. (Marked #28 on work list)

The structures including frames and beams on a sailing vessel must be in satisfactory condition and suitable for the intended service of the vessel in accordance with 46 CFR 177.330. During an inspection of the interior spaces, several transverse, overhead beams had significant checks that exceeded the standard tolerance in the following locations; Forecastle; First transverse beam forward of the mast. Each vessel must have on board an FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's hydrostatic release on the float free EPIRB expired in May 2019. Vessel restricted to 3 nm from shore until corrected.
