The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Vessel has sustained damage to the STBD bow, with 02 3in. X 4in. punctures approximately 8ft. above the waterline. Provide repair proposal and conduct repairs to the satisfaction of a USCG Marine Inspector. - 46 CFR 177.100

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including inspection and testing of all smoke and fire detection systems, including sensors and alarms. Vessel representative and crew were unable to prove annual testing of the fire detection system and alarm panel.

Resolution: Received, reviewed and accepted third party company's tech report attesting to the testing and satisfactory operation of the fire detection alarm panel.

Each ungrounded system must be provided with a suitably sensitive ground detection system located at the respective switchboard that provides a continuous indication of the circuit status to ground with a provision to momentarily remove the indicating device from the reference ground. Ground detection system test function did not function as designed and Marine Inspectors were unable to verify the ground detection system's functionality.

Resolution: Received, reviewed and accepted video and third party electricians report attesting to the proper operation of the main switch board's active ground detection system. Vsl has a ground detection system meetig 46 CFR 183.376.
