Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Upon inspection of all life rafts, PSCO found banding straps still attached from delivery installed, preventing proper inflation. SOLAS (20) III/20.2

Means shall be provided to control leaks of flammable liquids. PSCO identified excessive leaks on top and adjacent to the Primary Oily Water Tank, from the motor in the Elevator Machine Room, and from the oil filter on Generator #2. SOLAS (09) II-2/4.1.1

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. PSCO identified open switchboard with unapproved ventilation source in Elevator Machine Room. SOLAS (04) II-1/40.1.3

The machinery ... shall be so installed and protected to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. PSCO identified Skimmer for floating oil in Primary Oily Water Tank not installed IAW manufacturer's design. SOLAS (04) II-1/26.1

A low-level alarm for each hydraulic fluid reservoir ... shall be given on navigation bridge. No alarm for low-level hydraulic fluid given on navigation bridge for pump #1. SOLAS (04) II-1/29.12.2

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed, and installed as to withstand the maximum working stress to which it may be subjected. PSCO identified steam leak on pipe IWO purifier room above Low Sulfur HFO Settling Tank. SOLAS (04) II-1/33.1

Machinery spaces of Category A shall be adequately ventilated so as to ensure that when machinery or boilers therein are operating at full power in all weather conditions including heavy weather, and adequate supply of air is maintained. PSCO identified that manual louvers not open during operation of EDG despite clear signage to leave open for ventilation.

The Company should establish procedures to ensure that the ship is maintained in conformity with the Provisions of the relevant rules and regulations. The above technical deficiencies, though of such a nature that do not warrant the detention of the ship, still indicate a failure or lack of effective implementation of the ISM Code related to onboard maintenance protocols. An internal audit of maintenance procedures is to be conducted and corrective actions implemented within three months from 24Feb2025 as required by Paris MoU Exam conducted in Hamburg, Germany. THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT CONVENTION TO ENSURE THAT THE SHIP IN ALL RESPECTS WILL REMAIN FIT TO PROCEED TO SEA WITHOUT PRESENTING AN UNREASONABLE THREAT OF HARM TO THE MARINE ENVIRONMENT. PSCO NOTED DURING FUNCTION TEST OF THE EMERGENCY GENERATOR, VOLTAGE METER INDIACTED 320 VOLTS. PER MANUFACTURERS DATA PLATE, RUNNING VOLTAGE SHOULD BE 440 VOLTS.
