Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information:(1) Maximum allowable working pressure;(2) Date of manufacture; and(3) Date of the latest test required by 33 CFR 156.170. Vessel had hoses on board that did not match the hose record book. Prior to conducting cargo operation, vessel shall update hose records and provide proof of documentation to the satisfaction of a Coast Guard Inspector.

The last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be—(1) Painted in the sequence of red/yellow/red. The width of the red bands must be 0.1 meter (0.33 foot) and the width of the middle yellow band must be 0.8 meter (2.64 feet). Vessel's vapor control piping markings were faded. Prior to conducting cargo operations, vessel shall restore paint and markings to the satisfaction of a Coast Guard Inspector.

General information and introduction. This section of the response plan must include— (2) The name, address, and procedures for contacting the barge's owner or operator on a 24-hour basis. Vessel had outdated contact list as required by Vessel Response Plan. Prior to conducting cargo operations, vessel shall update Vessel Response Plan to the satisfaction of a Coast Guard Inspector.

The transfer procedures required by § 155.720 must be:(a) Available for inspection by the COTP or OCMI whenever the vessel is in operation;(b) Legibly printed in a language or languages understood by personnel engaged in transfer operations; and(c) Permanently posted or available at a place where the procedures can be easily seen and used by members of the crew when engaged in transfer operations. Vessel did not have transfer procedures for specific vessel. Prior to conducting cargo operations, the vessel shall obtain the correct transfer procedures and provide proof of documentation to the satisfaction of a Coast Guard Inspector.

All cargo tank hatches must be clearly marked in not less than three inch lettering “Danger—Keep Out,” and all hatch covers must be closed and dogged down in such a way that the hatch cannot be opened by the use of bare hands alone. Vessel had multiple hatches that did not have proper markings. The vessel shall properly mark all hatches and provide proof to the satisfaction of a Coast Guard Inspector.

The lights prescribed by these Rules shall, if carried, also be exhibited from sunrise to sunset in restricted visibility and may be exhibited in all other circumstances when it is deemed necessary. Vessel had permanently installed navigations that were inoperable. Restricted to daylight hours until vessel can demonstrate proper illumination of navigational lighting to the satisfaction of a Coast Guard inspector.

(a) Oil tankers and offshore oil barges with an overall length of less than 400 feet must carry appropriate equipment and supplies for the containment and removal of on-deck oil spills of at least 7 barrels.(b) The equipment and supplies must include—(1) Sorbents;(2) Non-sparking hand scoops, shovels, and buckets;(3) Containers suitable for holding recovered waste;(4) Emulsifiers for deck cleaning;(5) Protective clothing;(6) A minimum of one non-sparking portable pump with hoses; and(7) Scupper plugs. Vessel were missing items from spill kit. Prior to conducting cargo operations, vessel shall complete spill kit and provide proof to the satisfaction of a Coast Guard Inspector.

Each cargo tank of a tank barge must have liquid overfill protection arrangements that meets the requirements of 46 CFR 39.2007(2). Each cargo tank alarm must be equipped with an intrinsically safe high-level and overfill alarm. Vessel high-level and overfill alarms were not operable at the time of inspection. Prior to conducting cargo operations,vessel shall complete repairs to the satisfaction of a Coast Guard Marine Inspector. 46 CFR 39.2009(a)1 46 CFR 39.2007(a)

Each hose must be marked with the following: the name of each product for which the hose may be used for; or for oil products the words "OIL SERVICE". Each hose also must be marked with the following: Maximum allowable working pressure (MAWP). Marine Inspector identified multiple cargo hoses not marked properly with type of product and/or MAWP. Prior to cargo operations, provide the required information on all cargo hoses to the satisfaction of US Coast Guard.

Each cargo tank of a tank barge must have a high-level indicating device, unless the barge complies with 46 CFR 39.2009(a). The high-level indicating device must (1) Indicate visually the level of liquid in the cargo tank when the liquid level is within a range of 1 meter (3.28 feet) of the top of the tank. Marine inspector witnessed 1 meter gauging stick for cargo tank 1P broken. Replace to the satisfaction of Coast Guard Marine Inspector prior to conducting cargo operations. 33 CFR 39.2003 (b)

The lights prescribed in these Rules shall have intensity as specified so as to be visible at the following minimum ranges: a side light, 3 miles. STBD navigation light was observed to be inoperable. Vessel installed temporary light supplied from tug. Permanent repair to be made within 30 days to the satisfaction of a Coast Guard Marine Inspector.

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Barge sustained earlier damage to the gunwale of void #3 on the starboard side. Barge is authorized to discharge cargo to enact permanent repairs to the satisfaction of the US Coast Guard. 46 CFR 32.60-1

The following locations in the weather are Class I, Division 1 (Zone 1) locations and may have only approved intrinsically safe, explosion proof, or purged and pressurized electrical equipment, and through runs of marine shipboard cable if the location is: - (1) Within 10 feet (3 m) of: (i) A cargo tank vent outlet; (ii) A cargo tank ullage opening; (iii) A cargo pipe flange; (iv) A cargo valve; (v) A cargo handling room entrance; or (vi) A cargo handling room ventilation opening. Identified portable diesel powered pump installed within hazardous zone. Move/replace pump out of hazardous zone.

Each cargo tank of a tank barge must have one of the following liquid overfill protection arrangements: An intrinsically safe overfill control system. Identified overfill alarm device disconnected at individual tank junction boxes. Reconnect wires and bring into operation.

46 CFR 32.55-25: On all tank barges, subject to the provisions of this sub-chapter the construction or conversion of which is started on or after July 1, 1951, each cargo tank shall be equipped with a vent. The barge is equipped with a vapor collection system (VCS), however the installed pressure-vacuum (PV) valve is not in accordance with the approved type as outlined in the VCS Plan Review Information Sheet (PRIS) as approved by the Marine Safety Center (MSC).

Provide the Marine Safety Center approval letter for the structural deck support of the newly installed lube oil and potable water tanks and the newly installed lube oil tank piping. The barge may be operated under the conditions listed on the Certificate Of Inspection while waiting on MSC approval.

Wiring Harness on Prime Mover is not intrinsically safe, explosion proof, or purged and pressurized for a Class I Div I area. Ref D8(dp) Policy Letter 02-2007 CH1. Barge is allowed to operate until Aug 08 per policy ltr.
