PSCO received a report that the main engine failed to come online when attempting to go astern. Five (05) additional attempts were made, and propulsion was not able to come online.

Hydraulic power operated steering gear shall be provided with a low level alarm. Audible and visual alarms shall be given on the navigational bridge. PSCO observed no audible alarms for steering gear alarms.

Objective evidence indicates company has failed to report various "defects" to admin or RO. SMS records demonstrate various records of inop equipment being reported and acknowledged by company from a period of July 2024 to present with no record of flag/class being informed. An external audit is required within 90 days.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids.
PSCOs observed excessive oil leakage coming from the generator in the engine room. Vessels engineers were unable to secure leak.
