Each towing vessel must have either a self priming, power driven, fixed fire pump, a fire main, and hoses and nozzles and paragraphs (a) Through (d) of this section or a portable pump and hoses and nozzles. Portable pump did not self prime and pump water. Repair/replace to the satisfaction of attending inspector.

Records of drills and instruction must be maintained in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. Vessel has no records of drills conducted onboard. Complete fire and man overboard drills in presence of CG marine inspector.

Alarms must: (1) Be visible and audible at each operating station. The vessel’s bilge alarm does not have an audible component at the operating station. Install audible bilge alarm to satisfaction of CG marine inspector.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. The vessel is missing one lifebuoy and two floating electric water lights. Place additional lifebuoy and two lights onboard to satisfaction of CG marine inspector.

Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The flexible hose installed on the stbd main diesel engine, fresh water cooling discharge, was found punctured. Replace with new flexible hose for intended service.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The bulbs for the port and starboard running lights are loose and not secured at the appropriate height behind the lens. Make appropriate repairs and provide evidence of operation to CG marine inspector.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Alarms must be provided for Low fuel level, if fitted with a day tank. A low fuel level alarm, both visual and audible, shall be installed on the day tank.

Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The steering hydraulic hoses connected to the ram were found cracked and deteriorated. Replace with new hoses fit for intended service.

The main engines must be equipped with RPM gauges at the machinery location. Install RPM gauges for both the port and stbd main diesel engines inside the machinery space.

An exposed hazard such as rotating machinery, must be protected by a cover, guard or rail. Install cover, guard or rail around the stbd main diesel engine belt accessory drive.

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. Vessel does not have a general health and safety plan. Create plan and submit to CG marine inspector for inspection.

Each towing vessel must be fitted with a general alarm that: Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm - When Alarm Sounds or Flashes Go to Your Station” The vessel’s general alarm does not have a flashing red light installed in the engine room.

The emergency duties and duty stations required by this section must be posted at each operating station and in a conspicuous location in a space commonly visited by crewmembers. If posting is impractical, such as in an open boat, they may be kept onboard in a location readily available to the crew. Vessel does not have an emergency station bill. Create and post station in at operating station and in crew mess area to satisfaction of CG marine inspector

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Ventilation openings for the engine room do not have closure devices. Install closure devices to satisfaction of CG marine inspector.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Vessel is storing flammable liquids on deck. Remove flammable liquids or install approved flammable liquid storage cabinet to satisfaction of CG marine inspector.

Electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Electrical wiring throughout the engine room were installed with plastic zip ties and not adequately supported. Also, a spliced extension cord was found daisy chained to the lights, and light covers were missing. Inside the engine room, install metal supports for the overhead electrical wires, light covers and remove daisy chain and install fixed wiring to the satisfaction of the attending Marine Inspector.

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Vessel’s TVR/logbook does not contain required entries. Provide completed logbook entries in 30 days.

The portable fire pump must be self-priming and power-driven, with a minimum capacity of at least 300 LPM (80 gpm) at a discharge gauge pressure of not less than 414 kPa (60 psi), measured at the pump discharge. Install pressure gauge at pump discharge.

Portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers. Two fire extinguishers in the engine space were secured using electrical tape and bungee straps. Install approved fire extinguisher brackets.

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Vessel did not have required distress signals (3 night and 3 day). New distress signals were placed onboard.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel previously employed crew that had not undertaken a pre-employment drug test. Informed owner of requirement. Vessel is not use employ crew that have not received results from a pre-employment drug test.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the associated fleet that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
