Every tanker constructed on or after 1 July, 1998 shall be provided with the means to enable the crew to gain safe access to the bow even in severe weather conditions in accordance with resolution MSC.62(67). The vessel has multiple guard rails severely wasted, broken, and missing on the deck and walkway, preventing crew member to have safe access on the catwalk.

PSCO observed cargo deck catwalk deck grating, used for escape or access to firefighting equipment was made of a composite material. Vessel did not provide proof that material meets fire integrity standard for its service.
