74SOLAS(2009ED) II-2/4.1.5 50ac Ignition sources shall be seperated from combustible materials and flammable liquids. PSCO observed cover missing from explosion-proof lighting fixture in paint locker. Spares not readily available.

74SOLAS(2004ED) II-1/40.1.3 50ac Electrical installations shall be such that the safety of crew from electrical hazards will be ensured. PSCO observed headset near marine sanitation device with frayed wiring spares not readily available.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company[ISM 10.1]. In meeting these requirements the company should ensure that inspections are held at appropriate intervals[ISM 10.2.1] and any non-conformity is reported, with its possible cause, if known[ISM 10.2.2]. Ships on board maintenance manual[2M-0701] states fire detection systems are to be tested for proper operation monthly[FSS 4.2.2]. Ships SMS maintenance [Form 5.2.6] gives guidance for testing fire detection systems according to makers instructions which should be kept on boar. As delineated in deficiency #2 the ship failed to follow ships procedures for conducting routine inspections of the fire detection system[ISM 10.2.1] and making repairs of non-conformities. PSCO observed three smoke detectors in engine room (one in generator room)(two forward of the engine room) being held up by tape. Crew has not been testing heat/flame detectors and is unfamiliar with testing procedures. Ship does not have testing procedures or equipment. An internal audit of the ships SMS focusing on the testing of fire detection systems is required.

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the organization to ensure the reliability of fire-fighting systems and appliances. PSCO observed crew is not conducting monthly inspections and testing of the flame detectors on board the ship as required by their SMS maintenance procedures.

Fire protection systems shall be kept in good order so as to ensure their performance. Fire detection and fire alarm systems. PSCO observed crew was not able to test flame detectors in the engine room and cannot confirm their performance.

07111 - Fire Fighter's outfits shall comply with the fire systems code. Personal equipment shall consist of the following: Protective clothing of material to protect the skin from heat radiating from the fire and from burns and scalding by steam.
