WHILE INSPECTING THE STARBOARD SIDE LIFEBOAT, PSCO DISCOVERED THE EMRGENCY BRAKE RELEASE LINE INSIDE THE LIFE BOAT WITH APPROXIMATELY 02 METERS OF SLACK WHICH WOULD ALLOW THE SNSTALLED LINE WEIGHT TO MAKE CONTACT WITH THE LIFEBOAT NOT ALLOWING THE EMERGENCY BRAKE TO BE RELEASED. WHILE LOWEREING THE LIFEBOAT, THE CREW MANNED THE SLACK AS FOR IT TO BE A NORMAL PROCEDURE FOR LOWERING THE LIFEBOAT.

WHILE BRIEFING THE MASTER FOR A SECOND ATTEMPT AT A FIRE DRILL, PSCO WAS INFORMED THAT DRILLS WHERE SOMETIMES PERFORMED WITHOUT AN SCBA. THE FIRE TEAM DONNING AN SCBA FOR A DRILL IS REQUIRED AS PER THE VESSLE'S SMS CONTINGENCY MANUAL CHAPTER 6 SECTION 6.2.2.
VESSEL'S [name withheld] DID NOT KNOW HOW TO PROPERLY RELEASE THE INSTALLED FIRE SYSTEMS IN THE GALLEY NOR THEIR INTEDNDED PURPOSE. AS PER THE SHIP MANAGEMENT MANUAL CHAPTER 18 SECTION 18.8, THE [name withheld] FOR FAMILIARIZING HIMSELF WITH ALL EQUIPMENT RELAVENT TO THE PERFORMANCE OF HIS DUTIES.
