Objective evidence discovered in an expanded ISM examination revealed the vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by deficiencies 02, 03, and 04. The drills of the crew shall take place within 24 h of the ship leaving port if more than 25% of the crew have not participated in abandon ship and fire drills on board that particular ship in the previous month. When a ship enters service for the first time, after modification of a major character or when a new crew is engaged, these drills shall be held before sailing. The drills required were not held before sailing when a new crew was engaged, nor at any time during the following voyage. 74 SOLAS (20) III/19.3.2

Parties responsible for fire extinguishing shall be organized. These parties shall have the capability to complete their duties at all times while the ship is in service. The vessel’s crew lacked familiarity with shipboard fire extinguishing procedures and equipment. During the fire drill, Port State Control Officers observed a breakdown in communication resulting in a loss of situational awareness, and a lack of familiarity with required equipment. 74 SOLAS(20) II-2/15.2.1.3

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. When requested by the PSCO to perform a routine test of the deck foam monitors, the crew was unable to operate the system for 20 minutes due to unfamiliarity with the piping and valve configuration. 74 SOLAS (20) II-2/15.2.2.1

The deck line is a horizontal line 300mm (12 inches) in length and 25mm (1 inch) in breadth. It shall be marked amidships on each side of the ship, and its upper edge shall normally pass through the point where the continuation outwards of the upper surface of the freeboard deck intersects the outer surface of the shell. The required deck line is not marked on the port side of the vessel. ICLL 66/88 4

Each completed operation shall be signed by the officer or officers in charge of the operations concerned and each completed page or group of electronic entries shall be signed by the master of ship. PSCO identified numerous operations of discharge overboard or disposal otherwise of bilge water utilizing the Oily Water Separator that were not recorded in the Oil Record Book Part I. The operations not recorded in the Oil Record Book Part I were identified and recorded in the Oily Water Separator Oil Content Meter event log. An open venting system may consist of individual vents from each tank, or such individual vents may be combined into a common header or headers, with due regard to cargo segregation. In no case shall shutoff valves be fitted either to the individual vents or to the header. PSCO observed vessel transferring sludge from all sludge tanks to the residual oil tank in the cargo deck area.
