Vessel's No.2 cargo hold portside bulkhead plating was holed while loading scrap metal. Hole is approximately 80mm x 20mm and dented area is approximately 200mm x 200mm. Effected area is located approximately 7.79m from the tank top and is approximately 16.75m from forward bulkhead. -ICLL 2005(ed) Art. 15

Resolution: Received class report. 45cm X 45cm of the fractured plating was cropped out and inserted with new plate of 18" X 18"X1/2". Each welds were dye penetrant tested and then hydro tested and found to be in order.

Resolution: Class surveyor attended vsl and confirmed the deficiently insulated fuel oil lines were properly insulated with glass wool. The lines include the main engine return line, main engine suction line, and the boiler fuel oil pump unit line.
