A vessel that is exposed to salt water not more than 6 months in any 12-month period since the last examination or initial certification must undergo a drydock and internal structural examination at least once every 5 years. Vessel did not begin dry docking prior to deadline of 31JAN2026. Vessel is not permitted to operate until dry dock and internal structural exam is credited by USCG marine inspector. 46 CFR 137.300

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard. Vessel failed to obtain COI Renewal Inspection prior to certificate expiry. Vessel must schedule COI renewal after dry dock and internal structural exams are completed. Vessel is not permitted to engage in operations until new COI is issued. See 46 CFR 136.210 for renewal instructions. 46 CFR 136.200

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must:(1)Be maintained to ensure proper operation;(2) Be suitable for route and service; and (3) Have suitable propulsion controls to provide the operator full control at each operating station. Both main engines had exhaust leaks due to missing or damaged gaskets and damaged piping. Make repairs to the satisfaction of the Coast Guard. Renew lagging.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided:(1) Main engine low lubricating oil pressure;(2) Main engine high cooling water temperature;(3) Auxiliary generator engine low lubricating oil pressure;(4) Auxiliary generator engine high cooling water temperature;(5) High bilge levels;(6) Low hydraulic steering fluid levels, if applicable; and(7) Low fuel level, if fitted with a day tank. Bilge alarm in the lazarette was not operating. Make repairs to the satisfaction of the Coast Guard.

Each distribution panel or switchboard on a towing vessel must be:(1) In a location that is accessible, as maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. There was a coolant leak on the port generator and starboard main engine.

The frame or support of each semi-portable fire extinguisher must be secured to prevent the extinguisher from shifting in heavy weather. The semi-portable fire extinguisher was not secured or mounted to the vessel. Secure the fire extinguisher to the satisfaction of the coast guard.

Electrical components that pose an electrical hazard must be in an enclosure. There are various dead end wires on the bridge and engine room that need to be secured to the satisfaction of the Coast Guard.

Electrical components that pose an electrical hazard must be in an enclosure. Absent breakers in the panel in the engine room were not covered and were exposed. Provide acceptable covers for absent breakers to the satisfaction of the Coast guard.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Starboard bridge door would not open. Make repairs to the satisfaction of the Coast Guard.

Each lifebuoy on a towing vessel must:(4) Be marked in block capital letters with the name of the vessel. All ring buoy had missing or damaged lettering. Replace all damaged lettering to the satisfaction of the Coast Guard.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. There was a coolant leak on the port generator and starboard main engine.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Vessel had a through hole penetration that allowed water in. Provide repair proposal and complete repairs to the satisfaction of the Coast Guard.

Electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emertgency conditions. Electrical outlet in galley displayed a ground fault when tested. Repair and submit report to the MSU Port Arthur office.
