Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs are to made by next Dry-dock.

AIS Class A device. A vessel of 65 ft or more must have on board a properly installed, operational Coast Guard type-approved AIS Class A device.

A station bill must be posted by the master on a vessel of more than 19.8 meters (65 feet) in length having a Certificate of Inspection requiring more than four crew members at any one time, including the master. (b) The station bill required by paragraph (a) of this section must set forth the special duties and duty station of each crew member for various emergencies. The duties must, as far as possible, be comparable with the regular work of the individual. The duties must include at least the following and any other duties necessary for the proper handling of a particular emergency: No station bill was present.

(a) The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. None were found at the operating station.

General. Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. child life jackets need to be stowed properly.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. The portable and installed fire protection equipment is past the expiration date.

COD is expired. Any vessel of at least five net tons wholly owned by a citizen or citizens of the United States is eligible for documentation under this part. This includes, but is not limited to, vessels used exclusively for recreational purposes and vessels used in foreign trade.

Communication Act Safety Certificate is expired. Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Each item of lifesaving equipment determined by the Marine Inspector to not be in serviceable condition must be repaired or replaced. 04 ring life buoys on board had wasted service plugs, showing evidence of water intrusion. 46 CFR 176.808(b)

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. The engine room emergency lights were not operable. Vessel restricted to daytime operations and leaving engine room hatch open when crew members are within engine until emergency lighting is operable. 46 CFR 183.432

The owner, operator, or Master must destroy, in the presence of the Marine Inspector, each firehose found to be defective and incapable of repair. The fire hose on the main deck, starboard side ruptured at the connection to the nozzle. Firehose destroyed and removed from service, replacement to be installed. 46 CFR 176.810(c)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port side Propeller is cracked. 46CFR182.200(a)

A hatch exposed to the weather must be watertight. Water. starboard side forward watch is leaking. 46CFR179.360(a)

Footholds, handholds, ladders, and similar means provided to aid escape, must be suitable for use in emergency conditions, of rigid construction, and permanently fixed in position, unless they can be folded, yet brought into immediate service in an emergency. Ladder Satarboard side aft compartment has a broken weld. 46CFR177.500(m).

Vessel fixed bilge power pumps are unable to pump water from any of the 10 watertight compartments as required by 46CFR 182.520. The vessel will be restricted from carrying passengers until corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels Certificate of Documentation is valid until 31AUG20 but the original is not on board in accordance with 46 CFR 67.313. The vessel will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel 3 Ring life buoys have deteriorated retro reflective tape as required by 46CFR185.604(i). Replace with Type II retroreflective material approved in accordance with 46 CFR 164.018. The vessel will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel is missing a magnetic compass at primary operating station in accordance with 46 CFR 184.402. The vessel will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel missing 4 children life jackets as required by 46CFR180.71(b). The vessel is only allowed to carry children for the amount of children life jackets they have onboard and will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide a currently corrected coast pilot and light list extract for operating area as required by 46CFR184.420. The vessel will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels has dead end wire in the Starboard Transducer Room and does not meet the standard of minimizing accidental contact with electrical installations per 46CFR183.200(c) . The vessel will be restricted from carrying passengers after January 3, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel is over 65 feet and is currently operating with a AIS Class B device and it dose not meet the minimum standard that A self-propelled vessel of 65 feet or more in length, engaged in commercial service must have on board a properly installed, operational USCG Type-approved* AIS Class A device as required by 33CFR164.46(b)(1)(i). Vessel is required to either: 1. Install an operational USCG Type-approved AIS Class A OR 2. Keep the operational USCG Type-approved AIS Class B and operate under the provisions of 33CFR164.46(b)(2)(ii) which restrict the vessel to not operate at speeds in excess of 14 knots. If number 2 is chosen, a notation will be made in the routes in conditions portion of you Certificate of Inspection. The vessel will be restricted from carrying passengers after October 31, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

46CFR179.360(b)(2) Starboard side forward hatch was not attached to the hatch frame to prevent its loss.

2. 46CFR183.410(a) Starboard side fuel tank void Lighting fixture did not have a have a guard.

3. 46CFR177.500(m) Starboard engine room void ladder was missing a bolt and is not suitable for use in emergency conditions.

4. Remove sea valve's for inspection as required by 46CFR176.610(a)

01- Develop procedures to ensure an accurate passenger count in accordance with 46CFR 185.504. The Passenger count including crew must be communicated either verbally or in writing ashore and be available to the Coast Guard upon request no later than 28DEC2017

03- Provide a Marine Radio operators permit for the master [name withheld] no later than 28DEC2017. 47CFR80.159(e)(1)

Repair craack weld located on the stern portside.

It was reported that the STBD Shaft was out of alignment which is causing vibration to the machinery components. Prove proper repair to the STBD shaft and components there of to the satisfaction of the attending CG Marine Inspector. CG MI shall be notified when the vessel is out of the water for attendance and inspection of the damage.

Properly replace corroded Hydraulic hose compression connection fittings: 2 PORT & 1 STBD IWO steering systems. 46 CFR 182.720(e)(1)

All Vessels greater than 65 FT in length must have navigation lights that meet UL 1104 " Standards for Navigation Lights". The bulb for the stern light does not meet UL standards. Properly install light bulb that meet UL 1104 Standards. 46 CFR 183. 420

Vessels are required to carry adequate and up-to-date Chart (Chart 11510) and Coast Guard Light Lists. Extracts from the publications for the areas to be transited may be provided instead of the complete publication. Provide an updated chart for area of transit along with Light Lists. 46 CFR 184.420.

Emergency/ training for fire, man-overboard, and abandon ship drills have not been completed at least every three months. The last drill dates were held in Aug and Sep 2016 and records for the last two quarters were not available for inspection. 46 CFR 185.420(a)
