The following watertight bulkheads in number 2 void starboard side were found to have penetrations: a. bulkhead between the collision and void 2 with 1 hole in the outer side of the bulkhead due to incomplete weld. b. Bulkhead in void 2 forward of the fuel tanks with 02 holes due to incomplete weld. Provide a temporary means to cover the penetrations until next dry dock in September 2025. 46 CFR 171.111

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. The emergency broadcast placard was for a Fishing Vessel. No vessel name was on the placard.

The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS. A self-propelled vessel of 65 feet or more in length, engaged in commercial service. The vessel's length is 66.6 feet in length. Install an AIS Class A device required by 33 CFR 164-46(b)(1) or Class B device required by33 CFR 164.46(b)(2).

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. During the man overboard drill a crew member tending the safety line for the crew member at the bottom of the recovery platform was observed not wearing a lifejacket nor tethered to the vessel, this presents an unsafe operation that is not addressed by the Emergency Instructions.
