PSCO observed active leaks from HFO line connections to the main engine cylinders #1-8, 2 plastic containers collecting fuel leaking from HFO purifiers #1 and 2, oil soaked rags beneath HFO purifier #2 and a plastic cup collecting fuel oil beneath the boiler fuel oil supply line. 74 SOLAS (14) II-2/4.1.1

15109 - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO reviewed the Safety Management System and found that there was no timeline requirement to check the engine room for fuel oil leaks. The Safety Management System only required "periodic checks of machinery". Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementations of the vessel's SMS under the ISM Code. An internal audit is required within 90 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to the USCG when completed. 74 SOLAS (14) IX/3.1 ISM Code 10.1

The rudder angle indicator located in the steering space attached to rudder post does not reflect actual position reading on bridge. Reading is 3-5 degrees off on both pumps. 74SOLAS(04) I/11. Received class report dated 09Jul08 w/ due date of 09AUG08 stipulating use of 'calibration table for rudder angle indicator' until repairs are completed.

Lifesaving › Launching Appliances › Davit

During launch of port lifeboat, crew initially had difficulty lowering davit. At the time the boat had a 2 degree starboard list due to cargo ops. Crew successfully lowered/raised boat 2 times. SOLAS requires capability to launce with list of up to 20 degrees. Verify adequacy of launch arrangement. SOLAS 74/78 (2004 consolidated) III/13.1.2
