The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements must be met: ignition sources shall be restricted. The bilge primary tank was found open to the atmosphere in the engine room. Recommend developing procedures for sounding tanks.

PSCO observed active leaks from HFO line connections to the main engine cylinders #1-8, 2 plastic containers collecting fuel leaking from HFO purifiers #1 and 2, oil soaked rags beneath HFO purifier #2 and a plastic cup collecting fuel oil beneath the boiler fuel oil supply line. 74 SOLAS (14) II-2/4.1.1

15 - Safety Management Systems (ISM) › N/A - No Subsystem › Maintenance of the ship and equipment

15109 - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO reviewed the Safety Management System and found that there was no timeline requirement to check the engine room for fuel oil leaks. The Safety Management System only required "periodic checks of machinery". Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementations of the vessel's SMS under the ISM Code. An internal audit is required within 90 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to the USCG when completed. 74 SOLAS (14) IX/3.1 ISM Code 10.1
