CHUTE HAPPENS

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge, an audible alarm that sounds upon discharge, and a means to reset devices used to automatically shut down engines. Fireboy system not overriding properly as designed. Provide functioning fireboy override system with visual and audible alarms. 46 CFR 184.502

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 65ft in length carrying not more than 49 passengers provided that the pump is used to de-water not more than one watertight compartment; have a light or other visual means at the control point to indicate operation. Visual indicator at helm not operating for aft and forward bilge pumps. Provide visual indication at helm while bilge pump is in operation. 46 CFR 182.520(e)(2)

Except as provided in 80.13(c), stations must have an authorization granted by the Federal Communications Commission. Vessel could not produce current FCC station license during COI inspection. 47 CFR 80.405(a)

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Strainers must be leak free. Strainers must be the type of opening on top for cleaning screens. A drip pan fitted with flame screen must be installed under gasoline strainers. Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. Vessel did not have a drip pan installed. 46 CFR 182.455(b)(6)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB failed operational test. Provide operable EPIRB. 46 CFR 180.64

A hatch exposed to the weather must be watertight. Coaming/gasket material was missing or damaged on all hatches. Provide adequate coaming/gasket material to make watertight. 46 CFR 179.360(a)

FCC requires mariners that operate a vessel with more than 6 passengers to have a valid FFC Marine Radio Operator Permit(MROP). Vessel master did not have FCC MROP. Master shall provide MROP. 47 CFR 13.7

The arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of the COI giving consideration to provisions for a seaworthy hull. Hole located on seating area port and starboard side. Provide closure devices for holes. 46 CFR 177.100

As appropriate for the intended voyage a vessel must carry updated US. Coast Pilot and Coast Guard Light list. Nav pubs not on vessel. Provide nav pubs. 46 CFR 184.420

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. No evidence of crew training or supervisory personnel 60 minute training. Provide evidence of required training completed. 46 CFR 16.401(b)

Vessel lifering was not proper size.
