The vessel shall be made available for a Coast Guard COI Inspection immediately and the vessel may no longer be permitted to operate.

Closure devices must be provided for deckhouse or or hull penetrations, which open to the exterior of the vessel. Four of the five deck house doors that were tested on the main deck failed a chalk test. Chalk test ALL doors and hatches and make watertight/weathertight.

All seals/gaskets must be properly maintained to prevent leaks of combustible liquids. The gaskets for the supercharger-to-block and governor-to-block mating surfaces on both GenSet engines are leaking significantly. Make proper repairs.

A remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be installed directly outside the fuel oil supply tank. The vessel's fuel system has remote fuel shutoff valves, however they are installed before each engines fuel/water separator, not directly outside the fuel oil supply tank. That system was tested. The shutoff valve for the starboard engine failed to actuate. Install shutoff valves for each service line, directly outside the tank.

A fixed fire-pump must be capable of delivering water at a pitot-tube pressure of at least 50 psi. When tested, the fire-pump could deliver water at only 42 psi.
