Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such risks shall be taken to the satisfaction of the Administration. PSCO observed light installations on the cargo deck exposed wiring posing a risk of fire or explosion. 74SOLAS14 II-1/45.5.4

Every tanker shall be provided with the means to enable the crew to go in safe access tot he bow even in severe weather conditions. Safe access must be constructed of fire resistant/nonslip surface. Vessel was unable to provide the installed Fiberglass Grating meets fire standard requirements.
