Port side damage is as follows. Bow void starting at collision bulkhead forward 6' up rake slope sheet bottom plate and knuckle is pushed up in an area 4-5 foot wide. Bottom transition plate 4-5" forward of bulkhead 24-26" inboard of port knuckle. Bulkhead stiffener angle has punched hole in plate approximately 1/2" x 1-32. Starboard side damage is as follows 1st sidelong knuckle bracket aft headlog approximately at 9' draft mark 3 pin holes in weld

During each inspection or reinspection for certification, all wing voids, rakes, cofferdams, and other void spaces on tank barges must be opened and checked from on-deck for the presence of water or cargo indicating hull damage or cargo tank leakage. All wing voids had 3-5" of water in them throughout. Remove all water as required (CORRECTED)

Openings for fill and vent pipes must be on the topmost surface of a tank. There must be no openings in the bottom, sides, or ends of a tank except an opening fitted with threaded plug or cap may be used on the bottom of the tank for tank cleaning purposes. Clean-out on bottom of tank is equipped with a flange fitting. Apply flange shielding as required.

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. The following deficiencies were identified: 1) #2 STBD Wing Void - #4 Outboard side vertical hull frame - cracked weld on bottom - pick-up 2) #2 STBD Wing Void - #5 Outboard side vertical hull frame - cracked frame on top - crop and renew 3) #3 STBD Wing Void - #2 Outboard side vertical hull frame - cracked weld on top - pickup 4) #5 STBD Wing Void - #3 Outboard side vertical hull frame - cracked weld on top - pick-up 5) #5 STBD Wing Void - #3 Outboard side vertical hull frame - cracked weld on top - pick-up 6) #5 STBD Wing Void - #4 Inboard side vertical hull frame - missing weld at bilge frame - weld 7) Aft void - Starboard quarter tie beam second from top bent and broken from hull - crop and renew 8) #2 Port Wing Void - #4 Outboard side deck stiffener - cracked on top - apply relief hole, gouge out and weld or crop and renew. 9) #3 Port Wing Void - #4 and #5 Outboard side vertical hull frame - cracked weld on top - pickup For above deficiencies, apply proper repairs in accordance with NVIC 07-69.
