THE FIRE RESISTANCE OF DOORS SHALL BE EQUIVALENT TO THAT OF THE DIVISION IN WHICH THEY ARE FITTED, THIS BEING DETERMINED IN ACCORDANCE WITH THE FIRE TEST PROCEDURES CODE, DOORS AND FRAMES IN "A" CLASS DIVISIONS SHALL BE CONSTRUCTED OF SSTEEL, DOORS IN "B" CLASS DIVISIONS SHALL BE NON-COMBUSTIBLE. DOORS FITTED IN BOUNDARY BULKHEADS OF MACHINERY SPACES OF CATEGORY A SHALL BE RESPONSIBLY GASTIGHT AND SELF-CLOSING. IN SHIPS CONSTRUCTED ACCORDING TO METHOD IC, THE ADMINISTRATION MAY PERMIT THE USE OF COMBUSTIBLE MATERIALS IN DOORS SEPARATING CABINS FROM INDIVIDUAL INTERIOR SANITARY ACCOMODATION SUCH AS SHOWERS. PSCO OBSERVED ENGINEERING CONTROL ROOM DOORS AND ELECTRICIANS WORK SHOP DOOR TO ESCAPE TRUNK FAIL TO CLOSE AND LATCH. 74SOLAS(2004)II-2/9.4.2.1

THE OIL RECORD BOOK PART 1 SHALL BE COMPLETED ON EACH OCCASION, WHENEVER ANY OF THE FOLLOWING MACHINERY SPACE OPERATIONS TAKES PLACE IN THE SHIP...BUNKERING OF FUEL OR BULK LUBRICATING OIL. PSCO OBSERVED BUNKER DELIVERY NOTES FROM 22NOV2024 AND 23JAN2025 WITHOUT CORRESPONDING ORB ENTRIES. MARPOL ANNEX 1/17.2

NOT LESS THAN ONE HALF OF THE TOTAL NUMBER OF LIFEBUOYS SHALL BE PROVIDED WITH LIFEBUOY SELF-IGNITING LIGHTS COMPLYING WITH THE REQUIREMENTS OF PARAGRAPH 2.1.2 OF THE CODE. IN MEETING THESE REQUIREMENTS, THE COMPANY SHOULD ENSURE THAT- .1 INSPECTIONS HELD AT APPROPRIATE INTERVALS .2 ANY NON CONFORMITY IS REPORTED .3 APPROPRIATE CORRECTIVE ACTION IS REPORTED .4 RECORDS OF THESE ACTIVITIES ARE MAINTAINED THE TECHNICAL AND OPERATIONAL NATURE OF DEFICIENCIES PROVIDE OBJECTIVE EVIDENCE OF A FAILURE OF LACK OF EFFECTIVENESS OF THE IMPLEMENTATION OF THE ISM CODE. AN INTERNAL SAFETY MANAGEMENT AUDIT REPORT SATISFACTORY TO THE OCMI MUST BE SUBMITTED WITHIN 3 MONTHS. 74SOLAS IX/3.1 ISM CODE 10.2

In zones on open decks or non-enclosed spaces on the open deck within 3M of any cargo tank outlet, gas or vapor outlet, cargo pipe flange, cargo valves or entrances and ventilation openings to cargo pump rooms, and cargo compressor rooms; in zones on the open deck over the cargo area and within 3M forward and aft of the cargo area on the open deck and up to a height of 2.4m above the deck; in zones within 2.4M of the outer surface of a cargo containment system where such surface is exposed to the weather. The following must be installed: 1. Means of control shall be provided for opening and closure of ventilation dampers. PSCO observed the fire dampers for the number 2 and number 4 engine room fans fail to close after several attempts. 74 SOLAS (2004) II-2/5.2.2.1 30AC

Fire-fighting systems and appliances shall be in good working order and readily available for immediate use. PSCO observed the emergency fire pump discharge inadequate pressure at approximately one bar. 74 SOLAS (2004) II-2/14.2.1.2 30AC

THE FIRE RESISTANCE OF DOORS SHALL RE EQUIVALENT TO THAT OF THE DIVISION IN WHICH THEY ARE FITTED AND IN ACCORDANCE WITH THE FIRE TEST PROCEDURES CODE. PSCO OBSERVED THE POOP DECK MAIN STAIRWELL, POOP DECK CABLE TRUNK, UPPER DECK MAIN STAIRWELL, AND BOAT DECK TRUNK A CLASS SELF CLOSING DOORS, AS INDICATED ON THE VESSELS FIRE CONTROL PLAN, UNABLE TO PROPERLY CLOSE. 74 SOLAS (2004) II-2/9.4.2.1 17AC

All life-saving appliances and arrangements shall comply with the applicable requirements of the code. Hand gear handles or wheels shall not be rotated by moving parts of the winch when the survival craft or rescue boat is being lowered or when it is being hoisted by power. PSCO observed the safety switch for the hand gear handles on both the port side davit launched liferaft and the rescue boat inoperable. 74 SOLAS (2020) III/34 LSA Code 6.1.2.6

PSCO OBSERVED ROUTER SPLICED INTO EMERGENCY LIGHT FIXTURE ON BOAAT DEAK, DAISY CHAINED APPLIANCES IN THE SHIP/ENGINE OFFICE AND OFFICERS DAY ROOM. 3 SEVERLY CORRODED LIGHT FIXTURES ON THE AFT SUPERSTRUCTURE BULKHEAD. MULITPLE UNGROUNDED LIGHT FIXTURES AROUND THE SUPERSTRUCTURE, AND 2 VISIBLE ALARMS ON FORWARD SUPERSTRUCTURE BULKHEAD MISSING GROUND. 74 SOLAS (2004) II-1/40.1.3 17AC

In meeting these requirements, the company should ensure that: .1 Inspections are held at appropriate intervals .2 Any non-conformity is reported, with possible cause, if known; .3 Appropriate corrective action is taken; and .4 Records of these activities are maintained. The technical and operational nature of the deficiencies 02-12 provide objective evidence of a serious failure or lack of effectiveness, of the implementation of the ISM Code. A safety management audit carried out bu the Administration or the RO with a corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency and release from detention. 74 SOLAS (2020) IX/3.1 ISM Code 10.2 30AC

Gas detection equipment shall be so designed that it may be readily tested. Testing and calibration should be carried out at regular intervals. Suitable equipment and span gas for this purpose should be carried on board. PSCO observed crew testing fixed gas with a cargo sample. 74 SOLAS (2020) VII/13.1 IGC 13.6.6 17ac

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed multiple empty drums blocking access to the life buoy located near galley ventilation damper and davit launched lift raft on port side. 74SOLAS (2020) III/20.2 17AC

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met; means shall be provided to control leaks of flammable liquids. PSCO observed oil leaks and oil accumulation on all six main engine cylinders and on main engine fuel pumps 2 and 4. Additional leaks were found on all 3 auxiliary engine lube oil coolers, in the incinerator fuel oil circulation pump, and under the auxiliary engine quick closing valve. 74 SOLAS (04) II-2/4.1.1 17AC

Flammable liquid lockers shall be protected by an appropriate fire extinguishing arrangement approved by the Administration. PSCO observed approximately 10 gallons of gasoline stored in the safety locker #523 adjacent to the paint locker. 74 SOLAS (2004) II-2/10.6.3.2 17AC

Where "A" class divisions are penetrated ...such penetrations shall be suitably insulated by extension of the insulation at the same level of the division. PSO observed multiple "A" class cable runs in bridge deck cable truck to wheelhouse uninsulated. 74 SOLAS (2004) II-2/9.3.1 17AC

The condition of the ship and its equipment should be maintained to conform with the provisions of the Code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board or without presenting unreasonable threat of harm to the marine environment. PSCO received condition of class requiring permenant repair of temp sensor on cargo tank #1 port. 74 SOLAS (20) VII/31.1 IGC Code (93) 1.5.3.1

For the spaces listed in 13.6.7, Alarms should be activated for flammable products when the vapor concentration reaches 30% of the lower flammable limit. PSCO Observed that the fixed gas cabinet would not alarm when gas was applied in three separate locations: Port air lock (bottom), Starboard air lock (bottom), IG air lock (bottom). PSCO observed no vapor concentration percentage register in any tested location. 74 SOLAS(20) VII/13.1 IGC CODE(93) 13.6.10 30ac

The deck water spray system should be capable of covering all areas mentioned in 11.3.1 with uniformly distributed water-spray of at at least 101/m^2 per minute for horizontal projected surfaces and 41/m^2 per minute for vertical surfaces. PSCO observed 06 nozzle heads along the port side and 09 nozzle heads along the starboard side with no water discharge. Additionally, PSCO identified a 2 inch diameter hole above the nitrogen plant room spraying water from a pipe for the deck water spray. PSCO observed no deck water spray coverage for the number 2 starboard tank top or the starboard manifold number 2 vapor line. 74 SOLAS (20) VII/13.1 IGC Code (93) 11.3.2

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of the deficiencies below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit satisfactory to the OCMI is required prior to clearance of this deficiency. 74SOLAS(20) IX/3.1 ISM Code 10.1

The vapor space of each cargo tank should be provided with a pressure gauge which should incorporate an indicator in the control, position required 13.1.3. In addition, a high pressure alarm and, if vacuum protection is required , a low pressure alarm should be provided. PSCO observed the crew unable to produce any pressure or vacuum alarms on any tank. 74 SOLAS (20)VII/13.1 IGC Code (93) 13.4.1 30 AC

Changes in set pressures should be recorded in the ships log and a sign posted in the cargo control room, if provided, and at each relief valve, stating the set pressure. PSCO observed no set pressures at or around the MARVs for the #1 port/stbd or #2 port/stbd cargo tank. Additionally, no pressures were posted in CCR. 74 SOLAS (20)VII/13.1 IGC Code (93) 8.2.7 17 ac

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel. The means for securing these doors weather tight shall be of gaskets and clamping devices. PSCO observed starboard compressor room door missing gasket material and PSA room air lock doors unable to seal properly. ICLL Annex I Reg 12 17 ac

Where electrical equipment is installed in gas-dangerous spaces, or zones as provided in 10.1.4, it should be to the satisfaction of the administration and approved by the relevant authorities recognized by the Administration for operation in the flammable atmosphere concerned. PSCO observed multiple certified safe fixtures improperly installed, missing grounds, or broken. Fixtures were observed in compressor room and spread throughout cargo area. 74 SOLAS (20) VII/13.1 IGC Code (93) 10.1.5 17ac

Before the ship leaves port and at all times during the voyage, all lifesaving appliance shall be in working order and ready for immediate use. PSCO observed the freefall lifeboat's steering inoperable. 74 SOLAS (20) III/20.2 code 30AC

The function of the fixed fire detection and alarm system shall be periodically tested to the satisfaction of the Administration. PSCO observed vessel unable to test flame detector above purifiers due to lack of proper equipment. 74 SOLAS (20) II-2/7.3.2 CODE: 17AC

Pressure relief valves shall be set and sealed by a competent authority to acceptable to the administration and a record of this action, including set pressure, should be retained on board. Vessel could not provide set and seal documentation for relief valves. 74 SOLAS (20) VII/13.1 IGC Code (93) 8.2.5 Code 17ac

Cylinders must be always secured, and when not in use, must be stowed in a rack in an upright postiion, with the valve protection cap in place. PSCO observed 10-15 refrigerant bottles not in racks, missing caps, and unsecured in compressor room. 46 CFR 147.60(b)(1) code 17 ac
