Fire hoses shall be of non-perishable material and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. Identified 02 fire hoses from the main fire locker with raised O-rings in the couplings preventing a proper water tight seal. 74 SOLAS 20 II-2/10.2.3.1.1

Cabin and stateroom doors shall not require keys to unlock them from inside the room. Neither shall there be any doors along any designated escape route which require keys to unlock them when moving in the direction of escape. Found the port emergency escape door in the crew recreation area lockable with a key preventing escape. 74 SOLAS 20 II-2/13.3.2.6.1

The marking must enable passengers/crew to identify the routes of escape and readily identify the escape exits. Additionally, escape routs signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. Starboard emergency escape door in crew recreation area was not labeled as an emergency escape. Emergency escape on the outside of the crew recreation area points into the crew recreation area instead of towards the emergency escape to the right. 74 SOLAS 20 II-2/13.3.2.5.1

Tender #20 experienced a loss of propulsion while tendering passengers to shore. Cause of loss of propulsion was determined to be three burned fuses on the starboard engine fuel pump. Vessel crew replaced fuses and was able to get the tender to operate as designed.

Fire doors in main vertical zone bulkheads...the approximate time of closure for hinged fire doors shall be no more than 40s and no less than 10s from the beginning of their movement with the ship in the upright position. PSCO observed 75% of tested fire screen doors in passenger accommodation areas to close in under 10s. SOLAS 20 II-2/9.4.1.1.4.2

All life-saving appliances and arrangements shall comply with the applicable requirements of The Code. Lifeboat 19 would not start upon request during abandonship drill. SOLAS 20 III/34

Openings in 'A' Class divisions shall be provided with permanently attached means of closing which shall be at least as effective for resisting fires as the divisions in which they are fitted. Fire doors leading from muster stations to lifeboat embarkation stations have improperly installed sequencing bars which do not permit doors to close. Additionally, doors have door stops which cannot be remotely disengaged. 74SOLAS(14 Cons.) II-2/9.4.1.1 40ac

The means of escape shall be marked enabling PAX to identify the routes of escape. Outside decks 14-16 do not have markings clearly identifying emergency escape routes. 74SOLAS(14Cons.)II-2/13.3.2.5.1 40ac

The operator of a vessel with a capacity of 250 barrels or more of oil shall provide transfer procedures that meet the requirements of this part and part 156. Transfer procedures are not available. 33CFR155.720

The capacity of the required two electrical generating sets shall be such that in the event of any one generating set being stopped it will still be possible to supply those services necessary to provide normal operational conditions of propulsion and safety. Minimum comfortable conditions of habitability shall also be ensured which include at least adequate services for cooking, heating, domestic refrigeration, mechanical ventilation, sanitary and fresh water. At approximately 1537 HST, on 15 October 2017, while anchored off Lahaina, Hawaii, the vessel suffered a complete blackout of electrical systems caused by the loss of both generator sets which were online at the time of the incident.

Doors shall be self closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing pressure. Fire door 16.7.01 does not reach Solas requirements.

A fixed fire detection and fire alarm system shall be so installed and arranged as to provide smoke detection. 9 smoke detectors on deck 9 are inoperable.

Where the contents and use of a space are such that there is a doubt as to its classifaction it shall be treated as a space within a relevant category having the most stringent boundary requirement. Flame locker in workshop on deck 4 has more than daily use storage.

Prior to November 05, 2010 exam, show documentation of re-familization training for twenty (20) MES personnel.

Fire control plan currently under review by MSC. Vessel needs to provide a copy onboard.

Vessl does not have a current emergency escape plan onboard it is currently being reviewd by MSC.
