09209 - All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. A fresh water pipe was found with a leak that was spraying in the duct above the galley onto all electrical equipment with in the space. 74 SOLAS 04 II-1/45.1.3 16ac

14104 - Any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. PSCO observed the return line fitted with the three way valve duct taped and leaking water into the bilge. MARPOL 2017 Annex I/14.2 10c

07123 - Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO found a water mist nozzle with a blue ampule installed in a galley dishwashing space Deck 10 aft, plan states that a yellow ampule should be installed in that space. 74SOLAS 20 II-2/14.1.1 10c

07105 Fire doors in main vertical zone bulkheads,galley boundaries, and stairway enclosures shall be self-closing and be capable of closing with an angle of inclination of up to 3.5* opposing closure. Fire doors #10.009, 9.061, and 9.068 were found not capable of self closing. 74SOLAS(09)II-2/9.4.1.1.4.1 10c

04110 Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. The second coxswain of lifeboat #24 was not familiar with launching operations and could not identify the fuel shut off valve. 74SOLAS(20)III/19.2.1 10c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. The vessel’s port side funnel wing was damaged as a result of a fire and the integrity and suitability of the exhaust piping and associated support structures for DG’s 1,2,4,Boiler 2, and the emergency compressor have been compromised

11131 - Every crew member with assigned emergency duities shall be familiar with these duties before the voyage begins. Lifeboat# 25 crew members could not demonstrate familiarity with fire fighting systems onboard the lifeboat. Additionally, 03 crew members from the crew galley and 01 crew members from the aft main galley could not demonstrate nor explain fire fighting procedures for the galley they were assigned to. 74 SOLAS (20) III/19.2.1, Action: 16c

11131 - Every crew member with assigned emergency duties shall be familiar with theseduties before the voyage begins. Firs and second in command of life boats #12 and #19 and second in command of lifeboat #21 were not familiar with general vessel operations and/or launching procedures. 74SOLAS (20) III/19.2.1

09298 - Cylinders (used for containing haxardous ships' stores that are compressed gases) must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Multiple cylinders containing various compressed hazardous gasses were observed haphazardly secured on the secured on the aft mooring deck with rope or ratchet straps and not in racks. 46CFR 147.60(b)(1)

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in workingorde and ready for immediate use. Lifeboat #18 was found to have an inoperable VHF radio. Lifeboat #19 had a broken handrail. 74 SOLAS (20) III/20.2

07123- Fire Safety - Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. 01 fire extinguisher and 1 semi portable fire extinguisher were blocked by vessel stores. 74 SOLAS 20 II-2/14.2.1.2

07123- Fire Safety- Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions, shall be kept in good order so as to ensure their required performance if a fire occurs. Fire door 5.036 was unable to close and latch due to a privacy curtains installed behind them. 74 SOLAS 20 II-2/14.2.1.1.1

07101 - Fire Prevention - In addition to complying with the specific provisions for fire integrity of bulkheads and decks of passenger ships, the minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 9.1 and 9.2. Flammable liquid was observed being stored outside of a category 14 space. 74 SOLAS 20 II-2/9.2.2.3.1

02106 - Structural Conditions - The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure the ship in all respects will remain fit to proceed to sea. Lifeboat #18 was found to have a crack in structural frame supporting the cover of the vessel. 74 SOLAS 20 I/11

07105 - Fire Safety - The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. Fire door leading into category 14 space was found to have a gap in excess of 25 mm at the bottom of the door. 74 SOLAS 20 II-2/9.4.1.1.2

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Restaurants, lounges, and bars on deck 5 have twist locks in means of escape route.

Means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route. the markings must enable passengers to identify the routes of escape and readily identify escape exits. corridor leading out of the hospital space has eight fixed chairs blocking the low location lighting.

Small enclosed rooms in a space that have less than 30% communicating openings to that space are considered separate spaces. joiner room between carpentry and upholstery rooms had its 30% communicating openings covered.

The Administration shall satisfy itself that the general structural strength of the ship is adequate for the draught corresponding to the freeboard assigned. Multiple cracks were found in the outer tunnels of the three stern thrusters by divers originating in 2016. A recent crack was fount the inner tunnel of the stem thruster #3 causing minor water ingress, which has been permanently repaired. The crew, classification society and company have identified plans to repair the external cracks (50ab) **** Due to the nature of the damage and its location the above deficiency has been extended till the vessel's next dry dock February 2019.****

Passenger ship shall be equipped w/an automatic sprinkler systm complying w/the requirements of the FSS Code, the number & arrangement of nozzles shall be such as to ensure an effective average distribution of water at least 5 liters per square meter. Multiple storage spaces had items stacked above max lvl required for nozzle to achieve required spray pattern. SOLAS 14/II-2/10.6.1.1 FSS 7/2.1.1.2

0999 - Other (Safety in General) - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. There was a pinhole wastage on the sewage treatment pump discharge line. - 74 SOLAS (04) I/11a - 10c

Where emergency power is a generator, it shall be started automatically upon failure of the main source of power within a max of 45 seconds. The EDG failed to supply power to emergency switchboard upon normal power loss during annual load test. 74SOLAS09 CHII-I Reg42.3.1.2

Every ship when in a port of another contracting government is subject to control by officers duly authorized by such government. Ship must receive plan approval by marine safety center for modifications made to deck 9 and deck 12. 74SOLAS09 CHII-1 REG 42.3.1.2

Additional aids for escape shall be provided as necessary to ensur accessibility, clear marking, and adequate design for emergency situations. Child care room on deck 12 does not have adequate markings for escape. 74SOLAS04 CHII-2 Reg13.1.3

There shall not be any doors along any designated escape route which requires keys to unlock them. Emergency escape door on child care area locaed on deck 12 was found locked. 74SOLAS04 CHII-2 Reg13.3.2.6.1

Vessels #3 bow thruster shaft seal was leaking hydraulic fluid. System has been drained to eliminate risk of additional pollution. Repair to be compleated and attested too by class.
