After any survey of the ship under Reg 10 has been completed no change shall be made in structural arrangement. During mooring the vessel struck a fendering system causing an approximate 3 inch by 3 inch puncture through the vessel hull in way of starboard engine room.

Resolution: Witnessed sat temp repairs and recieved class report requiring permanent repairs by 01 Sept 16.
