At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: (h)Operational test of all emergency electrical systems. During test of emergency lighting observed the following issues with e-lights as marked/shown on FCP: -inoperable emergency lighting (3) in engine room -missing retro-reflective lighting strip on bottom step leading out of engine room -inoperable emergency light (1) near elevator, car deck. -inoperable lighted "EXIT" signs in main pax cabin.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: (7)Inspection and testing of smoke and fire detecting systems (including sensors and alarms) and fire confining appliances (such as fire screen doors and fire dampers).As prescribed on the approved fire control plan, the double leaf doors leading to the main passenger cabin on both #1 and #2 end are required to be self-closing doors. On each end, one of the two door leafs did not fully close when tested.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Both doors leading to the main passenger cabin on #1 and #2 end have deadbolt locks on the upper part of the door. If locked from the inside, passengers pushing on the normal crash bar door opening device would not be able to open the door and egress from the main passenger cabin.

MI observed 01 ring life buoy on the #1 end upper deck Type II SOLAS tape to be deteriorated by UV light and requires replacement.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. The expiration date of the battery may be marked on the battery in clearly legible letters or the owner or managing operator may have a record of the expiration date from the manufacturer of a battery marked with a serial number. MI observed floating waterlight on the #2 end of upper deck ring life buoy to have an expired battery which requires replacement.

The inset is located in the #1 End, void #3 on the stbd fwd end, between fr 16 & 17. The inset was supposed to be corrected at the last DDX but was overlooked due to the yard filing for bankruptcy. Needs to be corrected at next DDX.

Vessels representative has been notified and a proper permit must be acquired by 20MAY16. In the interim Vessel is required to follow the guidelines stated in the Washington State Ferry permit and procedure for portable Propane Cylinders.
