In meeting these requirements, the company should ensure that inspections are held at appropriate intervals, any non conformity is reported and corrective action is taken and records of these activities are maintained. Gauges throughout the engine room have not been maintained and calibrated as required. Many guages have discoloration of glycerin, and low glycerin levels. Rudder Angle Indicator in steering gear space is out of calibration. Conduct internal audit of SMS for maintenance and inspection of guages for machinery equipment as required by the manufacturer for the guages. 74 SOLAS (2024) IX/1, ISM Code Reg. 10.1 and 10.2

The CSR was updated, but still does not reflect the correct Owner to match the COD.

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel is required (03) ABs per the vessel's COI. One of the ABs had requested to be let go on 10Aug23 while the vessel was in Baltimore in mid September. A replacement AB was requested 10Aug23 through TOTE. That AB depart earlier than anticipated while in Davisville, RI due to the AB quitting early. The vessel was not able to obtain a replacement AB prior to sailing from Davisville, RI to Baltimore, MD due to the lack of availability of ABs. The vessel is scheduled to pick up a relief AB once in Baltimore. Vessel is authorized a one time transit from Davisville, RI to Baltimore, MD sailing short with (02) ABs.

Return to COI required crew compliment prior to departure from the next port of call, which is reported to be Charleston, SC.

The masters or individuals in command of all vessels, whether required to be inspected under 46 U.S.C. 3301 or not, are responsible for properly manning vessels in accordance with the applicable laws, regulations, and international conventions. The subject vessel currently only has 2 oilers onboard, less than the number of oilers required by the COI. Return to COI required crew compliment no later than listed due date.

The Officer in Charge, Marine Inspection (OCMI) will determine the specific manning levels for vessels required to have Certificates of Inspection by part B of subtitle II of title 46 U.S. Code(U.S.C.). The masters or individuals in command of all vessels, whether required to be inspected under 46 U.S.C. 3301 or not, are responsible for properly manning vessels in accordance with the applicable laws, regulations, and international conventions. On 28 JUN 2022, the vessel reported to the OCMI that it will sail short one (1) Oiler. Vessel shall return to full complement no later than 08 JUL 2022.

Starboard windlass motor is inoperable and unable to retrieve starboard anchor if lowered. Vessels in oceans service shall be fitted with anchors in general agreement with the Standards established by the American Bureau of Shipping or standards of other recognized classification societies with approval by the Commandant. Conduct repairs to satisfaction of CG marine inspector and class surveyor.

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. The main source of electrical pwer shall consist of at least two generating sets. Vessel sustained an unknown failure of the #2 generator while operating under normal conditions resulting in a loss of propulsion. 74 SOLAS (04 Const.) II-1/41.1.1

Cylinder #6 exhaust valve is not operating as per designed.

Power ventilation of accommodation spaces...and machinery spaces shall be capable of being stopped from an easily accessible position outside the space being served. The position shall not be cut off in the event of a fire in the spaces served. The EDG room has an electric ventilation fan that is not capable of being stopped from outside the EDG room.

The rescue boat has a faulty jet-drive transmission and is inoperable. Prove proper operation of the rescue boat prior to departure from port.

VDR needs to be updated to reflect new name. This includes AIS, LRIT, GMDSS and SSAS.

New COFR needs to be obtained to reflect name change.

Vessel has installed an alternate management system (AMS) for ballast water which is accepted by USCG and meets the requirements of 33 CFR 151.2026. At this time the unit is not functional. Per manufacturers correspondence letter, the servicing tech is not available but will attend the vessel at first availability in Japan on June 19. The vessel must demonstrate proper operation/functionality.

Vessel is to obtain Engine International Air Pollution Prevention Certificates (EIAPP) issued by Environmental Protection Agency (EPA) for all diesel engines. Vessel has EIAPP certificates issued by Lloyds Register on behalf of Sweden which the United States finds sufficient until US accepted EIAPPs are on board but in no case later than 01 June 2016

Under the Maritime Security Program (MSP) the Authorized Classification Society (ACS) is to issue certificates as indicated in NVIC 01-13. Prior to departure from port provide copies of all required certificates as indicated in Enclosure (2) 3.6 to NVIC 01-13

Vessel has installed a Alternative Management System which is accepted by the USCG and meets the requirements of 33CFR151.2026, however the unit does not function properly. Prior to the completion of the next scheduled drydock May 2016, the vessel must demonstrate proper operation to the attending USCG and provide a technician report. This does not excuse the vessel from proper BWM and reporting.
