The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that inspections are held at appropriate intervals, any non conformity is reported and corrective action is taken and records of these activities are maintained. Gauges throughout the engine room have not been maintained and calibrated as required. Many guages have discoloration of glycerin, and low glycerin levels. Rudder Angle Indicator in steering gear space is out of calibration. Conduct internal audit of SMS for maintenance and inspection of guages for machinery equipment as required by the manufacturer for the guages. 74 SOLAS (2024) IX/1, ISM Code Reg. 10.1 and 10.2

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel is required (03) ABs per the vessel's COI. One of the ABs had requested to be let go on 10Aug23 while the vessel was in Baltimore in mid September. A replacement AB was requested 10Aug23 through TOTE. That AB depart earlier than anticipated while in Davisville, RI due to the AB quitting early. The vessel was not able to obtain a replacement AB prior to sailing from Davisville, RI to Baltimore, MD due to the lack of availability of ABs. The vessel is scheduled to pick up a relief AB once in Baltimore. Vessel is authorized a one time transit from Davisville, RI to Baltimore, MD sailing short with (02) ABs.

Vessel has installed a Alternative Management System which is accepted by the USCG and meets the requirements of 33CFR151.2026, however the unit does not function properly. Prior to the completion of the next scheduled drydock May 2016, the vessel must demonstrate proper operation to the attending USCG and provide a technician report. This does not excuse the vessel from proper BWM and reporting.
